It was an exceedingly evil place, yet it also had the lowest rate of "malicious incidents" in the Shadowlands.
If you were a traveler willing to abide by the rules the Jailer had established for this land, staying in the Maw was even far safer than staying in Ardenweald—much less compared to Maldraxxus, where people "killed for fun," or Revendreth, where "sadists" were everywhere.
In fact, if "killing" or "evil deeds" alone were used as the criteria, then compared to the other realms, it was really hard to say which one deserved to be called "hell."
