Of the four, Ōnoki was the most badly injured. Nagato's black rods had pierced his shoulder, and he'd also crashed down from such a great height. He hadn't been able to defend himself with ninjutsu, and on top of that, he was already very old.
So when he hit the ground, Ōnoki lost consciousness immediately. If he wasn't treated in time, he might not make it.
Tsunade immediately ran over and lifted Ōnoki up.
As Konoha's finest Medical Ninja, Tsunade immediately administered basic treatment. She quickly confirmed that, given Ōnoki's current condition, he would need surgery to stabilize his injuries.
They had to get Ōnoki away from the battlefield and operate on him as quickly as possible. Otherwise, the consequences would be unthinkable.

Nagato no longer cared whether he lived or died. He simply wanted to kill Jiraiya and the others here, so that his sacrifice would have some meaning.
Meanwhile, Yu had captured Konan, but bringing her out now wouldn't accomplish anything. Nagato couldn't be stopped anymore.
