Kakashi took a slight deep breath beneath his mask, telling himself not to be nervous, to act natural and casual.
He casually raised a hand and waved to Shikaku, then carried a bag of medicine clearly bought here as he calmly walked out of the pharmacy, though inwardly he was desperate to leave.
It was nothing, nothing. He had merely been spotted by chance.
Good! Mr. Nara hadn't spoken to him!
With that, Kakashi left the pharmacy. Only after putting some distance between them did he quicken his pace.
Soon, he slipped into an alley, leaned against the wall, and took a deep breath.
He lowered his head and looked at the tonic medicine he had bought from the pharmacy.

As for why he had not let the pharmacy employees help? Wouldn't that have been much easier for someone as afraid of trouble as him?
What a joke! Did he, the Nara Clan head, not care about his reputation? Trouble was one thing, but he couldn't let ridiculous speculation like "the Nara Clan head can't perform anymore" spread just because he wanted to avoid trouble!
