Those were the lurking Hydra agents. Killing a few did not matter; it would even help establish his persona, letting him play the role of a mad scientist on the run and show that he was capable of anything.
Doom had originally wanted the fake Reed to flee to China, since that was S.P.E.A.R. Agency territory. S.H.I.E.L.D. had no authority to enforce the law there, and China did not play along with the Security Council at all. They were strong enough to protect themselves.
But Deathstroke rejected Doom's plan. Reed still had the identity of a scientist, and what was needed now was for him to flee, not defect across the ocean. That would trigger a new Cold War.
In the end, the route for the remotely controlled humanoid toy was decided. He would first dive across the Atlantic Ocean to Cuba, then swim from Cuba to Brazil, and finally disappear into the Amazon Rainforest.
