If they truly possessed the emotions of Little Helen's parents, then upon seeing Little Helen covered in wounds, his mouth full of scars, and hearing his babbling cries, they could never have remained so calm.

And as Instance NPCs, perhaps their purpose was simply to wait for Little Helen to awaken them, so they could give him an emotional pillar to lean on?
No one, including Angel, could understand what Little Helen was crying about. Yet there was not much resentment in his emotions. He was likely not speaking of the abuse he had suffered, but more of how dearly he missed his parents.
Perhaps Little Helen himself knew that the parents before him were fake, and that telling them of his suffering would serve no purpose.
