The Box and the Empress were both born within a dimension, but their depths were not necessarily below the apex of that dimension.

As for how this happened, there could be many possibilities.
The first was like Best Metal: its information was held by the Supreme Heavenly Ghost.
The Supreme Heavenly Ghost was originally above the Endless Dimensions, so what he created naturally surpassed the dimensions.
Bolo Ge's avatar was the same. Its body's Universal Trait was a 'Universal Trait' covering all dimensions, similar to the ultimate containment measure.
However, the Trait only required an avatar's corpse to appear within a dimension. That did not mean the corpse's absolute information was also within the dimension.
This meant its information existed at a higher boundary, making it impossible to observe.
It differed from the ultimate containment measure in this respect. Although the ultimate containment measure also covered all dimensions, Qingfeng allowed the creatures below to perceive it.
Bolo Ge's avatar, however, could not be perceived. He did not allow it.
In short, the creator was higher than the dimension and had merely placed the creation in a lower dimension.
