Tsukuyomi could not kill anyone. The last time Itachi fought another version of himself, it had only been to seize control of the body.
The two of them had fought for three days and three nights. Not only had their heads been bloodied, their bodies had been pierced with countless holes. In the end, even as they crawled across the ground, they still wanted to finish the other off.
He had survived even under those circumstances. How could the other side have gone completely silent just because he had been stabbed through with a blade, looking as though he had already been killed?
He had neither gained control of the body nor taken control of the entire Tsukuyomi World. Of course, the other side was not dead.
Uchiha Itachi surveyed his surroundings. Even his Sharingan could not spot anything out of place. He had no idea where the other side could be hiding.

He lowered his head and looked at Sasuke lying on the ground.
This Sasuke who had appeared in the Tsukuyomi World and been killed by his own hand had to be controlled by that other version of himself. He might even have been that person outright.
Yet Itachi had no intention of stabbing him a few more times. He did not want to do that, even if he merely wore Sasuke's face.
Itachi fell silent, and everything around him seemed to fall silent as well. He could hear only his own breathing.
I clearly remember that Uchiha Itachi and the Third Hokage had made an agreement beforehand. Everyone in the Uchiha Clan had to die, except Uchiha Itachi's younger brother: Sasuke Uchiha.
