Background / History
Childhood and Green Mountain Town
Xiao Yi Xian grew up without parents, wandering from place to place without a proper name. After learning medicine, she began treating illnesses and injuries. She eventually settled in Green Mountain Town because it seemed peaceful and supported herself through her medical skills 79.
Her patients were primarily mercenaries. Even men accustomed to crude behavior treated her courteously, and she became a respected member of the Myriad Medicine Pavilion's herb-gathering parties 78 79.
While gathering herbs near a cliff, she nearly fell to her death and was rescued by Yao Wan. She invited her rescuer to Green Mountain Town, beginning a close friendship in which she called Yao Wan “Sister Wan” and was affectionately addressed as “Xian'er” 78 80.
The Poison Body and Cave Expedition
Despite her desire to become an alchemist, Xiao Yi Xian lacked the innate qualifications. Her medical knowledge did not solve the more urgent problem of her Calamity Poison Body: poison powder she had prepared for self-defense triggered uncontrollable Poison Dou Qi within her 80 81.
Yao Wan told her about Xiao Yan before leaving town. Xiao Yi Xian subsequently recognized him from those descriptions and approached him. Their shared connection to Yao Wan provided an initial basis for trust 86 100 101.
She and Xiao Yan explored a hidden cliffside cave she had discovered. Mu Li and his mercenaries confronted them, but Xiao Yan killed Mu Li and defeated the others. Xiao Yi Xian summoned a Blue Eagle to carry them away 103 106.
Mu She's retaliation threatened both her and Green Mountain Town. Although she initially opposed Xiao Yan attacking him because of the risks and possible collateral damage, she ultimately supported Xiao Yan's nighttime assault by scattering poison from the Blue Eagle 110 111.
Cloud Empire and Reunion
Xiao Yan initially persuaded her not to travel to the Cloud Empire, and they stayed together in a safe location for a time. After they separated, however, she went there and founded the Poison Sect 165 240 249.
Her constitution granted extraordinary cultivation potential but continued to threaten her life. Having tried nearly every poison available in the Cloud Empire, she traveled to the Black Corner Region seeking unfamiliar poisons and information about a Poison King. Xiao Yan found her there after nearly two years apart; by then, her hair had turned white because of her poison body 240 241 244.
She initially refused to accompany him to Jia Nan Academy. His insistence that Yao Wan could help eventually persuaded her to go. At the academy, Yao Wan and Xiao Yan formed a Toxic Body Nascent Pill inside her, providing a means of restraining her toxins 244 246 247.
Jia Nan Academy and Xiao Yan's Disappearance
After struggling with her feelings and her concern for Yao Wan, Xiao Yi Xian confessed to Xiao Yan. He accepted their relationship while admitting his fear of disappointing her. She also offered to help him understand and pursue Yao Wan rather than demand that he abandon his other attachments 271 272 273 274.
When Xiao Yan disappeared beneath the Heavenly Flame Refining Qi Tower, she remained at the Inner Academy because she believed he would return. During the following two years, she protected the academy and retaliated against Black Corner Region leaders who had participated in the attack. Her killings caused several factions to collapse or lose their former standing 306.
Their reunion found her at Two-Star Dou Zong. She explained that she had restrained her cultivation because he had not been present to help control her toxins during breakthroughs 306.
Zhongzhou and the Evolved Poison Flame
While traveling to Zhongzhou, Xiao Yi Xian was separated from Xiao Yan and their companions by a Space Storm. She encountered Yao Wan first and explained the separation 352 353.
Yao Wan proposed nurturing a portion of Poison Flame within her constitution. Xiao Yi Xian refused to accept it if doing so would harm Xiao Yan; Yao Wan subsequently helped divide its origin safely. With Xiao Yan's assistance, she refined the origin and replaced her nascent pill with a growing internal flame 354 383 384.
After further seclusion, she approached Dou Zun and later forcibly entered the Star Region at Sacred Pill City. Her subsequent encounter with Xiao Yan completed the flame's evolution into the Calamity Poison Flame and raised both of them to Four-Star Dou Zun 479 490 491.
Return to Her Former Home
Much later, Xiao Yi Xian admitted that she had never returned to Green Mountain Town since leaving. Xiao Yan suggested a visit, and she traveled there with him and Yao Wan 953.
Finding no familiar faces initially left her heavy-hearted. At the Myriad Medicine Pavilion, an older shopkeeper recalled her kindness and reputation as a healer. His memories helped address her lingering doubts about whether she had been a good person. She gave him a Spirit Crystal and left feeling lighter 954 955 956.
Despite later responsibilities at the Medicine Tower, her preferred future remained modest: to become a little physician again and live peacefully with the family she had regained 1139.
Reincarnation Depictions
- Clinic encounter: In one of Xiao Yan's later reincarnation narratives, Xiao Yi Xian treats him after he initially holds a knife to her throat. Their shared orphanhood helps establish trust, and she grows accustomed to his assistance before his departure 625 626 630.

The danger of her constitution makes her increasingly pessimistic and reluctant to burden people she cares about. During her reunion with Xiao Yan, she tries to conceal why her hair has changed and fears that remaining near him could harm him 244.
Her loyalty can also become ruthless. She refuses power that might injure Xiao Yan, yet retaliates lethally against the factions involved in his apparent death 306 383.
In romance, she develops from hesitant and easily embarrassed into someone willing to confess and actively pursue intimacy.
