Background
Tsunade is the granddaughter of the First Hokage, the grandniece of the Second Hokage, and a disciple of the Third Hokage. Her lineage, reputation, strength, and political standing make her one of Konoha's foremost candidates for Hokage; her hemophobia is initially the primary obstacle to that position 23 26.
During the Second Shinobi World War, Oguri Hayashi served under her command. When he defects from Konoha and steals the Scroll of Seals, Tsunade considers his betrayal partly her responsibility and attacks him personally 19.
The deaths of two people close to her leave Tsunade grieving and her chakra visibly erupting in rage. Her hemophobia remains severe enough that blood can incapacitate her and prevent escape, though she refuses Hayashi's offer to cure it in return for her Yin Seal 25.
Appearance
Tsunade is a blonde kunoichi commonly recognized by her golden hair and Konoha forehead protector. Her prominent Senju heritage and status make her immediately identifiable even outside Konoha 26.
- Blonde hair.
- Usually wears a Konoha forehead protector.
- Described as radiant after overcoming her hemophobia 85.
- Bears a purple, rhombus-shaped Sage Mode Yin Seal on her forehead after advancing her cultivation 86.
Personality
Tsunade is deeply loyal to Konoha and reacts fiercely to those who abandon it for personal survival or power. Hayashi's refusal to fight in shinobi wars angers her because she believes Konoha cannot endure if its ninja reject their duty to the village 24 25.
She is also pragmatic and observant. While Hayashi holds her captive, she quietly reattaches her own arm, feigns harmlessness to gather intelligence, and studies his techniques for future use 23 28. Her time in Takigakure challenges her assumptions about the role of ninjutsu in civilian life, leaving her contemplative rather than immediately dismissive of Hayashi's reforms 34 35.
After returning to Konoha, Tsunade conceals her intentions from Danzo and handles political tension with controlled politeness. Her recovery from hemophobia restores both her confidence and her willingness to return to frontline responsibility 40 51.
Abilities & Skills
Super Strength Technique
Tsunade channels chakra through her muscles to produce explosive physical force without hand seals.
- Her chakra-enhanced punches can smash combat puppets and devastate the ground on impact 20.
- She can seize and slam High Ninja opponents to the ground through raw Taijutsu and explosive power 27.
- Her refined technique releases chakra through her feet for sudden, high-speed movement 40.
- Hayashi initially counters her because her combat methods are comparatively direct and predictable 20.
Medical Ninjutsu
Tsunade is regarded as the Ninja World's foremost medical ninja and the founder of Konoha's medical-ninjutsu system 50 71.
- Reattaches her own dislocated arm while held captive 23.
- Removes embedded shuriken, disinfects wounds, uses anesthesia, and heals critically injured patients in the field 71.
- Develops Anesthetic Jutsu into a standard technique for medical squads; its effects can interfere with surgery or combat 71.
- Her medical corps allows Konoha to treat wounded ninja on the battlefield rather than abandoning care to rear-area facilities 140.
Chakra Scalpel
A chakra blade technique incorporated into Tsunade's close-range combat style.
- Applies precision medical chakra control offensively 92.
- Can be combined with Body Disturbance Charge to produce crippling effects on contact 92.

An A-rank technique that disrupts an opponent's nervous system with chakra.

- Develops the Sage Art Yin Seal later used to restrain Jugo's unstable Sage Chakra 205.
- Her seal-based endurance is described as surpassing Naruto's Sage Mode sustainability 86.
Katsuyu Summoning
Tsunade can summon Katsuyu of Shikkotsu Forest.
- Uses Katsuyu as a source of knowledge on Sage Jutsu 33.
- Maintains contact with Konoha through Katsuyu while in Takigakure 33.
Story Role
Capture and Takigakure
When Hayashi returns to Konoha with the stolen Scroll of Seals, Tsunade confronts him as his former commander. He blocks her signature punch, defeats her in close combat, dislocates her arms, and takes her hostage to secure his escape 19 20.
Hayashi transports Tsunade to Takigakure, where she remains for three years. Though captive at first, she observes his rule closely, studies his training methods, and witnesses Takigakure's transformation into a rapidly growing village where civilians learn practical ninjutsu for work and daily life 32 35.
Recovery and Return to Konoha
Tsunade's hemophobia is cured after her years in Takigakure. She demonstrates her restored strength by eliminating attackers who exploit knowledge of her former weakness, then returns to Konoha to lead its medical forces against the Land of Wind 40 50.
Her return is celebrated by Konoha's population, who view her as a vital protector during the coming war. The Third Hokage considers assigning her leadership of the Western Front against Chiyo 50 51.
Medical Commander and Sage Combatant
As Western Front commander, Tsunade saves numerous Konoha ninja from Sand poison and secretly treats the injured Sand kunoichi Pakura 71. She subsequently broadens her medical repertoire into direct battlefield techniques, making her a more versatile close-quarters combatant 92.
Tsunade later joins the Sannin in operations against Madara's forces, including the recovery of Uchiha clan items and the defeat of White Zetsu 116. She also participates in the Ninja World Protection Association's meetings and guides visiting leaders through Takigakure 100 108.
Instructor and Medical Legacy
Tsunade becomes one of the founding instructors of Takigakure's Ninja Academy, teaching Medical Ninjutsu to students from member villages of the Ninja World Protection Association 140. Her discipline is considered one of Konoha's strategic treasures because it has transformed battlefield survival and rehabilitation across the Ninja World 140.
Rin Nohara is selected as a candidate for Tsunade's advanced medical training, while Sakura Haruno is set to become her disciple. Karin Uzumaki is also established as Tsunade's disciple and successor in Super Strength and medical-ninja training 144 206 251.
