Background / History
Tsunade is the granddaughter of Hashirama Senju, the First Hokage, and a disciple of Hiruzen Sarutobi. Her lineage gives her considerable political standing: during the Fourth Hokage succession debate, several village leaders support her specifically because she is a Senju and Hashirama’s granddaughter 13.
Her younger brother, Nawaki, and her lover, Dan Katō, died during the Second Shinobi World War. Their deaths left her deeply disheartened and unwilling to remain in Konoha. After the war, she left with Dan’s orphaned niece, Shizune, whom she took into her care, and wandered through the Land of Fire 13 16.
Appearance
Tsunade is a woman in her early thirties with long, light-golden hair 15 16.
- Hair: Straight and tied into a long ponytail behind her head 15.
- Forehead: A prominent diamond-shaped mark between her brows 15.
- Clothing: A short-sleeved kimono beneath a green jacket, cropped pants, and open-toed high heels 16.
Personality
Tsunade associates Konoha with personal loss and has little desire to resume village life or pursue political office. While traveling, she spends her idle time drinking and gambling. She openly declines the Hokage position rather than accepting it out of obligation to her lineage 15 16.
Her reluctance to return does not erase her concern for others. She cares for Shizune after Dan’s death and comforts Biwako Sarutobi following Hiruzen’s murder. When war threatens Konoha, she remains to help administer and defend the village 16 17 25 31.

Hemophobia
Following Nawaki and Dan’s deaths, Tsunade develops a fear of blood severe enough to make her faint at its sight 11 13.
- Her condition is cited as a reason she cannot shoulder the Hokage’s responsibilities during the succession crisis 13.
- Sakumo excludes her from frontline command when choosing forces to confront enemy nations 23 24.
- When the Third Kazekage attacks, she stays to guard Konoha rather than risk collapsing on the battlefield 31.
Relationships
- Hashirama Senju — Grandfather. Her descent from the First Hokage is a major source of her standing within Konoha 13.
- Nawaki — Younger brother, killed during the Second Shinobi World War 13 16.
- Dan Katō — Deceased lover. His death contributes to her withdrawal from village life 13 16.
- Shizune — Dan’s niece and Tsunade’s young traveling companion. Tsunade takes responsibility for her after she loses her last relative 16.
- Hiruzen Sarutobi — Teacher. His murder prompts her return to Konoha, and she insists that the search for his killer continue 15 19.

Story Role / Major Arcs
Hokage Succession and Return to Konoha
- Succession debate: Sakumo nominates the absent Tsunade for Fourth Hokage. Although several attendees favor her, the advisors cite her grief, hemophobia, and the urgency of choosing a leader to justify appointing Sakumo instead 13.
- Return: Tsunade arrives with Shizune and Tonton after learning of Hiruzen’s murder. She returns despite her aversion to the village because she considers it a public and personal obligation 15.
- Unsolved murder: After hearing that investigators have found no substantial clues, she considers whether an exceptionally powerful member of the older generation could be responsible, but identifies no suspect 16.
- Refusal of office: Sakumo offers her the Hokage position after her return. She declines, citing his greater seniority and reputation as well as her own lack of interest 16.
Hiruzen’s Funeral and the Official Explanation
- Comforts Biwako at the Sarutobi residence, then stands beside Hiruzen’s coffin with his family and Orochimaru during the funeral. She appears sorrowful but does not cry 17 18.
- Approves publicly blaming Danzō to give frightened villagers a known target and preserve stability. Her agreement is conditional on continuing the search for Hiruzen’s actual killer 19.
- Her approval carries political weight: other clan leaders hesitate to endorse the explanation before learning her position and that of Hiruzen’s son, Sarutobi Ken 19.
Wartime Administration
- Sakumo leaves Tsunade and Shikata in charge of Konoha, with orders to prepare against the Land of Wind and prevent Kakashi and Jiraiya’s three disciples from leaving the village 25.
- When the Third Kazekage advances, she accepts Shikata’s plan to delay the Sand army while preserving sufficient forces to defend Konoha 31.
- Sakumo’s victory over A and Killer B relieves her concern for the southwestern border, but she remains worried about Shikata’s confrontation with the Kazekage 32.
- On Sakumo’s return, she supports holding A, Killer B, and Dodai in the rebuilt Root base under Hyūga guard. She then urges him to reinforce the northwestern front 36.
- Organizes a welcome for the victorious forces returning from the Sand campaign and reports that Jiraiya and Orochimaru have also gained the upper hand 39.
