Yoru returned from the front lines and treated Mikoto after the death of her comrade destabilized her emotionally. Their early bond was described as sibling-like; Mikoto entrusted him with clan knowledge and advanced techniques for battlefield survival 5.

At Nawaki's grave, Yoru awakened both a one-tomoe Sharingan and an unidentified second Kekkei Genkai. Tsunade diagnosed the resulting rejection as a Bloodline Disease requiring continuous monitoring and medicine; misuse could shorten his life 10 11. His condition gave him a legitimate reason for repeated contact with Tsunade, which the Uchiha leadership turned into political leverage.
