It stood in the center of the Seireitei, near the First Division Barracks, and had suffered some collateral damage during the earlier battle between Kenma and Yamamoto.
However, a large Kido Barrier protected the headquarters, keeping the damage relatively light.
When Kenma arrived, he saw members of the Onmitsukidō bustling in and out, never stopping for a moment.

Kenma walked along the brightly lit corridor with Shisui behind him, heading straight for the prison deep underground.
Hearing Kenma's words, Shisui recalled the bustle he had seen upon entering the Onmitsukidō and nodded in agreement.
"He is the Second Hokage, after all-a highly capable ninja."
Looking at Kenma ahead of him, Shisui recalled their conversation on the Hill of the Executioners.
After a moment, he could not help asking, "My lord, you said earlier that you used one of my eyes a few years ago. May I ask who you used it on?"
"Oh, that? There's no harm in telling you."
Kenma glanced at Shisui, a smile curling his lips.
"I planted him beside Kisuke Urahara as a spy to monitor his movements."
"Kisuke Urahara?"
Shisui had not expected to hear that name here.
Kenma nodded, a trace of mockery crossing his face.
"Kisuke Urahara is a dangerous man. How could I possibly leave him unchecked?"
"He thought fleeing to the World of the Living would put him beyond my control, but he underestimated me. I've known exactly what he's been doing all along."
