Background and History

The Medical Department’s name predates the Dragonfall Dynasty’s fall. Its predecessor was a Dragonfall government agency responsible for recovering Pestilence remains; the present organization was founded after the Great Calamity began and the dynasty collapsed. Plague Exterminators who inherited the power of the world formed it to protect humanity 739.

Historical accounts credit the Medical Department with treating patients before the Great Calamity, working on epidemic prevention during it, and originating modern plague-extermination techniques 725. Over the following millennium, it became the authority above the Six Nations, while its inner workings remained largely hidden from outsiders 919 767.

Organization and Territory

• Seat of power — The Medical Department controls Sovereign City on Banquet’s End Continent in Formula World. Its Depths are a restricted zone that even Six Nations leaders cannot enter without permission 724 767.
• Ten Families — Its authority is concentrated in ten families, whose members possess advanced technology, exceptional combat power, and the ability to bestow special powers known as titles 937 980.
• Family roster — The families are named as Luo, Gu, Fu, Chu, Chi, Zhou, Bai, Weng, Ruan, and Si. Their standing can change: Ruan Yin later declares that the Zhou Clan no longer qualifies and subsequently confers new Ten Houses titles 1022 963 1066.
• Leadership — Ruan Yin is the Medical Department’s Holy Lord, though a campaign against external forces is approved by a Round Table discussion without her participation 968 969.
• External agents — The Weng Family acts as a public mouthpiece, while the Bai Clan holds much of the authority over Abstinence, a suppression force 934 725.

Methods and Resources

• Medical technology — The department possesses some of the world’s most advanced plague-extermination technology. Its resources include specialized instruments, title-bestowing methods, and the strategic anti-Pestilence weapon Fire God Mountain 739 980 1023.
• Rebirth — Duanmu Qiannian is revealed to be a vessel created through the department’s Rebirth technique 670.
• Medical oversight — Department personnel monitor Xuanyuan’s practical Medical Examination for cheating 218.
• Military forces — Its campaigns employ family Plague Exterminators, outside recruits, and patients. During the assault on Ultimate Bliss, the Luo and Si clans field roughly six thousand core members alongside a much larger outside vanguard 1027.

Major Conflicts

After Formula World’s seals open in World Calendar Year 333, the Medical Department announces its return and proposes the Special Residency Treaty, under which its Plague Exterminators would be stationed in the Six Nations for epidemic prevention. The offer attracts support where medical resources are scarce, but leaders in Chengyun and Ultimate Bliss see it as interference in their governments 934.

The department subsequently moves against the Six Nations, presenting its campaign as treatment for a world whose leaders have betrayed humanity. Its forces enter Xuanyuan and launch offensives on multiple fronts 945 1022. At Ultimate Bliss, the Luo and Si families suffer a major defeat: both family heads fall, and Fire God Mountain is captured. The Ten Houses withdraw their troops afterward 1062 1066.

The Patient Association openly names raising its blades against the Medical Department as its ultimate goal. Zhou Zhi likewise opposes the department’s abuse of ordinary people, even while recognizing its Plague Exterminators as human allies against disease when their interests coincide 906 925 1040.
