Background / History
Tsunade belongs to the Senju Clan and is the granddaughter of the man who founded Konoha. She trained under Hiruzen Sarutobi alongside Jiraiya and Orochimaru, with the three becoming known as the Sannin. She also fought in wars before the events of the story 12 14 16 103.
After losing her younger brother and lover, Tsunade withdrew from the shinobi world’s conflicts. Years of traveling left her weary of Konoha’s disputes and unwilling to inherit its leadership crisis. Orochimaru later exploits her grief by offering to resurrect both loved ones in exchange for treating his damaged arm 13 14.
Appearance
- Forehead mark: Bears the Hundred Healings Mark, which lights up when she activates its power 16.
- Physique: Has tremendous vitality and physical strength, which she identifies as foundations of her Strength of a Hundred Technique 103.
- Official attire: Wears Hokage robes when presiding over the village’s leadership 62.
Personality
Tsunade combines a strong sense of medical responsibility with an explosive temper. She protects Yūya from Orochimaru because he is her patient, but repeatedly responds to Yūya’s insults with punches that send him through walls or out of buildings. During the confrontation outside town, her accumulated humiliation turns into violence against enemies, gamblers, Yūya, and Jiraiya alike 13 16 31.
Her gambling habit produces debts and financial misconduct. She prolongs Yūya’s supposed treatment to charge Konoha additional medical expenses, then uses those funds for gambling and drinking. Later, she disguises a psychiatric evaluation as a payment voucher to obtain research funding while providing him a subsidy 13 14 31.
As Hokage, she listens to advisers and increasingly prioritizes the younger generation’s safety. This concern coexists with a willingness to suppress direct challenges to her authority by force. Yūya’s criticism nevertheless pushes her to become more diligent in her duties 31 62 82 103.
Abilities & Skills
Medical Ninjutsu
Konoha Hospital describes Tsunade as the finest medical ninja in the shinobi world, with abilities far beyond its own staff 12.
- Diagnosis: Examines Naruto and finds no physical explanation for his nearly catatonic condition, identifying an unexplained spiritual disturbance instead 14.
- Complex surgery: Assesses Rock Lee’s operation as having a fifty-percent chance of recovery, with possible death if it fails. She subsequently performs the surgery 31 34.
- Medical instruction: Teaches Sakura the essentials of medical ninjutsu over the two-and-a-half-year interval, then plans to formally accept her as an apprentice 81 82.
- Limits: Orochimaru’s arm is physically intact but cannot function because its soul was sealed, distinguishing his condition from an ordinary bodily injury 13.
Super-Strength Taijutsu
Tsunade’s physical power overwhelms Yūya during their early confrontations, even when he uses Haki to anticipate or withstand her attacks 14 16.
- Striking power: Punches Yūya into a wall and later knocks him out of the Hokage Building 14 31.
- Close-range control: Grabs Yūya after he evades her first punch, then knocks him unconscious with a body slam 16.
- Battlefield strength: Defeats Orochimaru and Kabuto after activating her seal 16.
- Teaching requirements: Plans to pass her super-strength taijutsu to Sakura. She rejects Yūya as a prospective apprentice partly because he would not commit to chakra training 82.
Hundred Healings Mark / Strength of a Hundred
Activation of Tsunade’s forehead mark floods her body with emerald-green chakra and restores her ability to fight during the confrontation with Orochimaru 16.
- Rapid regeneration: Immediately heals the minor injury caused by Yūya’s Armament Haki-coated punch 16.
- Sustained recovery: Holds down a rampaging Naruto while continuously repairing the damage inflicted by Nine-Tails chakra 44.
- Physical foundation: Tsunade attributes the technique’s foundation to her Senju vitality and bodily strength, rather than exceptionally large chakra reserves 103.
Leadership and Diplomacy
- Consultative judgment: Accepts Kakashi’s advice to cooperate with Kazahana Dōtō and improve relations with Sunagakure after Hiruzen’s death 31.
- Threat assessment: Identifies chakra-less Yūya as Konoha’s best available counter to Urashiki’s chakra-absorbing abilities 67.
- Protective deployments: Sends younger ninja away from Konoha during its internal political crisis 103 104.
- Military response: Orders Konoha’s deployed ninja toward the Land of Lightning during the Kumogakure confrontation 115.
- Negotiation: Proposes compensation from Kumogakure for Konoha’s losses while excluding the Hyuga Clan from compensation 116.
Hemophobia
Tsunade initially suffers from severe fear of blood. Kabuto deliberately sprays blood onto her, causing her body to freeze and leaving her vulnerable to attack 15.
- Combat vulnerability: The sight of blood drains her strength and prevents her from fighting effectively 15.
- Documented recovery: During the same battle, overwhelming anger overrides her fear and enables her to activate the Hundred Healings Mark. The supplied events do not establish whether this permanently cures the condition 16.
Relationships
- Miki Yūya — Patient, frequent antagonist, and later a paid protector for dangerous assignments. She alternates between defending him, empathizing with him, and violently punishing his provocations; she also acknowledges his unusual combat talent 13 14 31 103.
- Jiraiya — Fellow Sannin and longtime associate. He recruits her for the Hokage position, brings her intelligence, and helps assess threats to Naruto and Konoha 14 67 81.
- Orochimaru — Former teammate whose resurrection offer exploits her grief. Their remaining personal bond does not stop her from protecting Yūya or defeating him in battle 13 16.
- Hiruzen Sarutobi — Former teacher. His death and ruined reputation leave her conflicted, while his administration’s unresolved problems become burdens of her own tenure 14 82.
- Shizune — Attendant who cares for Yūya in Tsunade’s absence and warns others about her hemophobia 15.
- Naruto Uzumaki — Patient and aspiring successor. She treats him, restrains his rampages, and later orders protection for him, but badly injures him when he intervenes in her confrontation with Yūya 14 44 62 134.
- Sakura Haruno — Medical-ninjutsu student and intended formal apprentice. Tsunade praises her chakra control, medical aptitude, and ability to learn 81 82.
- Kakashi Hatake — Trusted source of mission intelligence and diplomatic advice; she also asks him to explain Lee’s surgical risks to Guy 31.
- Danzō Shimura — Political rival who exploits unrest to weaken her authority. She withholds information about Urashiki from the wider leadership because she expects Danzō to misuse the crisis 35 64 67.
- Rock Lee — Surgical patient whose prospects prompt her to argue that a meaningful life need not depend on becoming a ninja 31 34.
- Adult Sasuke Uchiha — Time-traveling informant. His arrival gives her renewed hope that Konoha survives and Naruto’s aspirations eventually bear fruit 67.
Story Role / Major Arcs
Recruitment and Return to Konoha
Konoha Hospital sends Yūya to Tsunade after failing to find the hidden injuries he insists he has. She uses his extended stay to finance gambling, but draws a firm boundary when Orochimaru threatens him 12 13.
Jiraiya arrives with the mentally incapacitated Naruto and asks her to become Fifth Hokage. She agrees to help Naruto but initially refuses the office. Yūya’s confrontation with Naruto moves her to recognize the suffering Konoha has ignored 14.
Her encounter with Orochimaru becomes a chaotic battle involving debt collectors and captured companions. After overcoming her blood-induced paralysis through rage, she defeats Orochimaru and Kabuto. She returns to Konoha with the group in Chapter 18 and is established as Hokage by Chapter 31 15 16 18 31.
Early Hokage Tenure
Tsunade inherits diplomatic weakness, medical crises, and widespread distrust of the leadership. She accepts Kakashi’s recommendations for rebuilding foreign relations while handling Lee’s dangerous surgery and Yūya’s compensation claim 31.
Following the Sound Four’s infiltration, Danzō and the elders attempt to make her bear sole responsibility for village security failures. Yūya redirects the resulting protest against the entire leadership, forcing Tsunade, Danzō, and both elders to bow and apologize publicly 35.
When Yūya later nominates Naruto and Shikamaru to replace the older leadership, the confrontation turns violent. Tsunade, Jiraiya, and Shikaku overwhelm the challengers, leaving all three injured outside the shattered office 62.
Urashiki and the Training Interval
Learning that Urashiki can absorb chakra, Tsunade and Jiraiya conclude that Yūya’s unconventional strength offers their best chance of resisting him. She keeps the threat secret from Konoha’s leadership and declines to hear detailed accounts of the future, drawing encouragement from adult Sasuke’s presence instead 67.
During the subsequent two-and-a-half years, she trains Sakura extensively and becomes more active as Hokage under Yūya’s continued criticism. Jiraiya’s return brings renewed concern over the Akatsuki 81 82.
Hyuga Crisis and Kumogakure Conflict
Before Hiashi’s final confrontation, Tsunade urges him to return safely if he cannot bring Neji home. Hiashi later interprets her restraint toward the defectors as an effort to make amends for the village’s and Main Family’s failures. He also possesses a Gelel-derived pill she gave him that restores his condition at the cost of certain death 98.
After Hiashi dies, she sends Naruto, Sakura, and Hinata to protect Shion and pays Yūya three million ryō to take Konohamaru away while intercepting Moryo’s army. Her request that he take his time leads Yūya to conclude that she is removing the younger generation before a deadly political purge; this remains his interpretation of her preparations 103 104.
She mobilizes Konoha against Kumogakure, then summons Danzō and the Hyuga leaders to discuss recovered intelligence and a compensation settlement 115 116.
Renewed Akatsuki Threat
After the Akatsuki captures the Two-Tails and Eight-Tails jinchūriki, Tsunade orders Naruto protected. His returning companions realize that Obito may already have supplied the organization with Nine-Tails chakra 134.
She subsequently convenes an emergency leadership meeting to discuss Konoha’s defenses, its response to the Akatsuki, and participation in the Five Kage Summit 135.
