Background / History

War service and the medicine trade

Tsunade fought alongside Jiraiya and Orochimaru against Hanzō during the Second Shinobi World War. Hanzō bestowed the title Sannin upon the three survivors; although they had been defeated, Konoha’s ninja regarded their survival as evidence of exceptional strength 8.

When Karn approached the Sannin in the Land of Rain, Tsunade recognized the importance of his ready-made medicines. Konoha relied largely on herbal preparations, and wounded soldiers were dying because its medical personnel and supplies could not meet demand. She pledged her name as a Konoha jonin to guarantee his safety and agreed to trade, becoming the Trading System’s bound partner for that world 6 7.

She returned to camp with Karn and Orochimaru, presented the medicines to the war council, and supported testing them before a bulk purchase. After their effectiveness was confirmed, she escorted Karn to Konoha, arranged accommodation in the Senju Clan Compound, and introduced him to the Third Hokage 8 9 10 11.

Karn’s explanations of battlefield first aid also influenced her early reform ideas. She recognized that ordinary ninja could learn suturing, bandaging, and emergency treatment much faster than they could become Medical Ninja, making systematic first-aid instruction a practical means of reducing battlefield deaths 8.

Medical-system reform

Tsunade began addressing the shortage of medical ninja through both practical staffing changes and formal training proposals. She hired Konoha women to assist medical personnel and learn bandaging and stitching, reducing the hospital’s workload 12.

Her initial appeal to Hiruzen Sarutobi lacked a concrete implementation plan. Following Karn’s advice, she enlisted Yano and Aosuke to help draft a proposal while she organized Medical Ninjutsu by difficulty and application. Her proposed system included selection standards, assessments, surgical instruction, and a reproducible learning method 23.

Nawaki’s severe injuries made the reforms more urgent. Tsunade saved his life through an exhausting operation, but permanent damage left him unable to continue as a ninja. She believed better battlefield medical coverage could have prevented that outcome 24.

Hiruzen rejected her proposal at a jonin meeting. Karn subsequently advised her to begin with inexpensive measures, such as assigning two medical ninja to teach fifth- and sixth-year Academy students. Over the following year and more, her reputation increasingly rested on her medical work rather than the Sannin title 28 29 41.

The reforms subsequently progressed along the lines Karn had recommended. Even while working at Orochimaru’s experimental base, she periodically returned to Konoha to supervise their implementation. Konoha Hospital gained additional intern medical ninja, and Tsunade eventually had three students requiring daily instruction 52 57 76.

Medical instruction became a regular Academy subject from the second year onward, with fourth-year assessments identifying students suited to becoming Medical Ninja. Hiruzen and the senior leadership accepted this arrangement because it could increase the supply of medical personnel without additional spending 119.

Her administrative responsibilities remained substantial. After returning from the battlefield and spending three days drinking and gambling, she required Nonō and another assistant’s help to clear her accumulated hospital paperwork over a further three days 171.

Nawaki’s recovery and Extremis research

Tsunade continued researching regenerative Medical Ninjutsu after work, seeking a way to restore Nawaki’s damaged bodily functions and nerves. She also blamed herself for giving him their grandfather’s necklace, believing his excitement had contributed to his failure to notice the trap 32 33.

Karn later offered her research on the Extremis Virus, whose potential included regrowing limbs and restoring Nawaki’s ability to become a ninja. Tsunade judged it superior to the Medical Ninjutsu she had mastered in some respects and proposed combining it with Hashirama’s cells. Before committing, she sought Mito’s permission to use her grandfather’s cells 42.

She defended Karn when Konoha’s leaders proposed investigating him, rejecting attempts to obtain his formulas through coercion. Hiruzen refused research funding but requested progress reports; Tsunade resented the village’s expectation of benefiting without contributing 43.

As the experiments approached success, she told Nawaki that he would recover and gain tremendous power, then collected his cells for the personalized serum. She remained cautious about proceeding after only one successful case and waited for Orochimaru’s broader testing 52 53.

Once all remaining experimental subjects had adapted to the New Extremis Virus Serum, Tsunade brought Karn and Nawaki to Orochimaru’s base. Although she initially wanted Nawaki treated at home, she accepted Orochimaru’s argument that the base’s monitoring equipment made it the safer location. She injected both men, whose transformations finished after approximately thirty minutes 55.

Nawaki recovered and acquired Wood Release, fulfilling the objective that had driven her research. Orochimaru took responsibility for his mission experience, while Tsunade was assigned to oversee his Wood Release training. His recovery did not mean she immediately accepted his wish to return to dangerous missions; she instead supported keeping him home to familiarize himself with his new abilities 57.

Conflict with Konoha’s leadership

After a Cloud Ninja spy attacked Karn and Kushina, Tsunade abandoned her laboratory preparations to check on them. Karn refused her protection and asked to leave Konoha, so she vouched for him before the Security Force and escorted him out 46.

Kushina’s account implicated an ANBU member and raised questions about Minato Namikaze’s involvement. Tsunade confronted Hiruzen with Orochimaru, whom she agreed to admit to the research project. Afterward, Mito warned that Tsunade would soon bear responsibility for protecting both Nawaki and Kushina. Tsunade resolved to gain authority through medical reform rather than continue compromising with the village leadership 46 47.

When Karn returned, she accepted his demand for a new underground laboratory outside Konoha with restricted access. Their collaboration continued despite his decision to limit future visits to the village 48.

She subsequently persuaded him to visit Konoha again and offered accommodation at the Senju Clan Compound for his safety. Although she understood that Kushina had likely been brought to the village as Mito’s successor as the Nine-Tails jinchūriki, she did not disclose that identity to him 52.

Tsunade attempted to reconcile Karn with Konoha by offering protection from the leadership and access to Wood Release techniques if he joined the village. Karn refused unless the leadership changed and Tsunade or Kushina became Hokage 55.

When another war threatened, Hiruzen again relied on her to secure Karn’s medicines. She angrily pointed out that the leadership had driven him away and was now making her responsible for restoring contact, but still agreed to try because Konoha remained her home 76.

Her relationship with Karn later suffered when she questioned his intentions after hearing hostile gossip. She sought to repair the damage by purchasing medicine in a personal capacity and giving him the medicines and formulas she had developed. Nonō investigated the people who had influenced her and secretly sent Karn her findings without Tsunade’s knowledge 90 91.

Tsunade continued defending Kushina against village pressure, including confronting Hiruzen after Kushina complained about her treatment. She also asked Karn to investigate the daimyo’s funding cuts and agreed to cooperate with his newspaper’s coverage of the Sannin 118 122.

Command against Kumogakure

Hiruzen appointed Tsunade commander-in-chief of the Land of Hot Water front during the war against Kumogakure. Initially unable to halt the Third Raikage’s offensive, she repeatedly withdrew her defensive lines and requested reinforcements. Kushina and additional clan ninja were eventually dispatched after her requests 157.

Tsunade and Nawaki together became capable of holding the Third Raikage at a stalemate, occasionally putting him at a disadvantage when their coordination was particularly effective. The effort required her full attention, preventing her from using Katsuyu to heal other soldiers during those engagements. Their accomplishments increased the reputation of the Senju siblings beyond Tsunade’s individual Sannin title 160.

She conveyed orders to reject Kumogakure’s demands and expel its forces from the Land of Fire. Her front received more than seventeen hundred personnel, with further assistance expected from Kusagakure and Takigakure. She also persuaded Kushina to contribute more than fifty healing potions by telling her that Karn had returned to Konoha 160.

When negotiations failed and fighting resumed, the Third Raikage fell into Tsunade’s ambush. Kushina opened the engagement with a Tailed Beast Bomb, after which Konoha’s forces gradually surrounded the Cloud Ninja 168.

The Sannin’s political divergence

Tsunade had no personal interest in becoming Hokage and regarded Nawaki’s eventual candidacy as a reason not to oppose Orochimaru’s succession plans. Nevertheless, she objected when Orochimaru described the expansionist philosophy he had taught Nawaki and his intention to eliminate threats to Konoha before they could develop 117 173.

Jiraiya urged her to support Minato instead. She refused, pointing out that doing so would mean abandoning her own brother’s future ambitions. The disagreement exposed the widening ideological divisions among the Sannin 173.

When Jiraiya decided to investigate the daimyo despite the risk of becoming a missing-nin, Tsunade tried both persuasion and threats to stop him. After he left anyway, she angrily regretted not having beaten him into the hospital first and declared that she would no longer involve herself in the dispute 182 183.

She later refused to join Jiraiya’s investigation of Karn’s human experiments. Upon learning that the subjects were criminals, she regarded the work as something she could understand, given her own experiments for Nawaki and Tobirama’s precedents. She warned Jiraiya not to abuse her reluctance to report a former teammate who was now a missing-nin 232.

Despite her conflicts with Hiruzen, his eventual death saddened her because he had once been her teacher 293.

Naruto’s birth and civilian treatment

Tsunade was assigned to oversee Kushina’s delivery personally. After Naruto’s birth, she thoroughly examined him and reported his health to Kushina, rebuking Karn for criticizing the newborn’s appearance 216 220.

Following the attack on Konoha surrounding the birth, she and Nonō went to the hospital to treat the wounded. Tsunade took responsibility for the injured civilians and summoned Katsuyu, while Nonō handled more seriously injured ninja. Recognizing that she had neglected Nonō’s large-scale healing training, she promised to arrange a summoning contract with Katsuyu afterward 220.

Pregnancy, Yoko, and withdrawal from Konoha

Tsunade became increasingly dissatisfied with Konoha’s political atmosphere despite its prosperity. She felt that the village had ceased to resemble the familial home envisioned by Hashirama and had become an association held together by mutual interests. Although she repeatedly wanted to travel, Orochimaru opposed allowing a Medical Saint to wander where rival villages might target her 223.

After a drunken incident led to intimacy with Karn, Tsunade felt guilty toward Kushina and sought a long-term assignment outside the village. Orochimaru refused, citing her responsibilities and her plans for Nawaki’s training. Karn’s pressure during their encounters included using bioelectric stimulation that left her physically unable to resist effectively 224 225 232.

While seeking advice about Jiraiya’s claims and the Great Toad Sage’s prophecy, she visited Shikkotsu Forest. Katsuyu initially mistook the powerful life force within her for Tsunade’s own development before identifying it as the life force of an unborn child 229.

Tsunade briefly considered ending the pregnancy but rejected the thought because she could not bring herself to extinguish her own child’s life. She decided to give birth and considered Shikkotsu Forest a suitable refuge, while planning to avoid Nawaki’s visits there. A subsequent self-examination confirmed that she was approximately one month pregnant 230.

With her borrowed money exhausted, she sought financial help from Xia Chuan and asked her not to inform Karn. Xia Chuan nevertheless contacted him out of concern. Karn found Tsunade, confirmed the pregnancy, and brought her by Apparition to his residence in Kawaki Town despite her insistence that the child belonged to the Senju Clan and had nothing to do with him 232.

Tsunade later lived outside Konoha with their daughter, Yoko. As an inexperienced mother, she learned child-rearing by trial and error and depended on Nonō’s frequent visits for help. When Karn returned after two years, she was happy to see him but greeted him with an angry kick and reproached him for his absence 277 278.

Enhancement and return to the village

Karn urged Tsunade to return with Yoko, who was nearly three years old, and promised to take responsibility for their years apart. He strengthened Tsunade with the Extremis Virus, T-virus, Hashirama Cells, and Woods-Woods Ability Serum, giving her a greatly enhanced physique and control over trees 278.

Her new constitution also removed the physical limitations that had previously prevented her from training in Shikkotsu Forest Senjutsu. She planned to pursue advanced study there once Yoko’s arrangements were settled 278.

After Karn told Kushina about Tsunade and Yoko, he brought mother and daughter to Konoha. The village’s ninja celebrated Tsunade’s return, assuming that her two-year absence had been caused by a classified mission. In the Hokage’s office, Hyuga Hiashi recognized the family situation and withdrew, also clearing the surrounding ANBU so that they could speak privately 279.

Tsunade explained her absence to Nawaki, who had feared that something had happened to her. Although he found the situation difficult to accept, he recognized her genuine happiness and stopped dwelling on it after a familiar blow from his sister. He subsequently addressed Karn as his brother-in-law 279 280.

She took Yoko around the village, prompting speculation about her husband’s identity. As Konoha prepared to relocate to New City, Tsunade worried about the agreement requiring her to move into the Daimyo’s Mansion and especially about facing Kushina 280 288.

Her protective attachment to family did not always align with Nawaki’s responsibilities as Hokage. After a later dispute over detaining someone suspected of treason, she admitted understanding the political necessity but remained angry at the way her younger brother had addressed her 289.

Future Heroic Spirit incarnation

During the Holy Grail War, a future incarnation of Tsunade appeared as a Heroic Spirit contracted to Rin Tohsaka. She revealed that she was Karn’s wife and that she came from the future. Although she had attempted to conceal her identity, Karn discovered it through mind magic. On reuniting with him, she refused to change Masters 341.

With Rin’s magical energy, two Command Spells, and magic gems supporting her, Tsunade used a refined version of Creation Rebirth to overwhelm Gilgamesh and Cú Chulainn. She survived Gáe Bolg piercing her heart, removed the weapon herself, and defeated Cú Chulainn with one punch 347.

Family reunions and interworld expeditions

Tsunade later summoned Karn to meet reanimated members of her family, including Hashirama and Tobirama. Karn promised to resurrect five relatives once research into the Pure Land advanced sufficiently, with Tobirama agreeing to assist the research while the others waited 368.

She accompanied Karn and Hermione on the Douqi Continent. After the Miteer Auction House sale, she took Purple Heart Barrier-Breaking Pills with them, quickly defeated robbers outside Jia Ma Holy City, and departed toward the Magical Beast Mountain Range aboard Karn’s Tailed Beast Nine-Headed Dragon 377.

When the Ancient Bodhi Tree attacked their group and the Chakra God Tree, Tsunade communicated with it and agreed to help heal its injuries. Karn refined its Bodhi Seeds into Bodhi Clear Qi, and the tree began accelerating the Chakra God Tree’s growth while taking Tsunade inside itself 406.

Tsunade accompanied Zi Yan to East Dragon Island, where she rejected the elders’ proposal to wait years for Zi Yan to reach Dou Sheng. After precisely striking and immobilizing Qingshan and Qiyou, she secured their agreement to gather forces for a campaign against the other three dragon islands 417.

When Zi Yan discovered that Zhu Kun was her father, she rejected him and clung to Tsunade, declaring Tsunade the only person she would acknowledge as family 439.

Tsunade was later among those who watched the Ancient Bodhi Tree undergo its Heavenly Sovereign Tribulation. The tree survived with Karn’s assistance and took human form as Bodhi 457.

Appearance

Personality

Tsunade is outspoken, quick-tempered, and willing to challenge authority. She openly criticizes Hiruzen’s rejection of medical reform and refuses to let Konoha’s elders target Karn without evidence of a threat. Her loyalty to the village does not extend to accepting every action taken in its name: she despises Danzō’s use of disguised Root operatives to massacre civilians and manipulate the war 8 28 43.

Her concern for family drives much of her research. She works late into the night for Nawaki, promises to restore his future as a ninja, and protects Kushina when the girl describes being ostracized at school. That same desperation leads her to accept human experimentation despite personal reservations; she participates in the research but avoids taking an active role in discussions about procuring subjects 32 38 43 50.

Drinking and gambling are her two major recreational passions. She also enjoys chocolate and cake, sometimes taking Kushina’s portions and pretending to sleep to avoid being confronted. Nevertheless, she can set aside leisure for sustained hospital work and research 16 23 32 43.

Her gambling causes significant financial problems. By the time Jiraiya prepared to investigate the daimyo, she owed roughly three hundred million ryō at Karn’s casino and had unsuccessfully attempted to conceal herself behind other identities. Her spending later left her dependent on loans while preparing for motherhood 182 230 232.

Although she is not personally ambitious for the Hokage position, she is deeply invested in Nawaki’s future and refuses to support Minato at his expense. Her family loyalty coexists with discomfort over Orochimaru’s influence on Nawaki’s political beliefs 117 173.

Tsunade can be influenced by emotion and gossip, as shown when she questions Karn’s motives without first resolving her suspicions. She is also capable of trying to make amends, approaching him afterward with her pharmaceutical results and seeking to repair their relationship 90 91.

Her views on experimentation remain qualified rather than wholly permissive. She dislikes human experimentation as a medical practitioner but accepts the use of heinous criminals who would otherwise be executed, drawing comparisons to her own efforts to save Nawaki and Tobirama’s research 232.

Motherhood exposes an unfamiliar, anxious side of her. Despite her medical expertise, she initially struggles with the discovery of her pregnancy, worries about Kushina’s reaction, and later needs Nonō’s help in raising Yoko. She nevertheless decides to protect the child, avoid unsuitable missions, and give up alcohol during pregnancy 230 277.

Temporary fusion

Using the Potara Earrings, Tsunade fuses with Hermione into Senju Hermione. The fusion combines Tsunade’s close-combat power with Hermione’s magical knowledge, allowing effective defenses and counterattacks against magic that had previously frustrated Tsunade 433.

The fusion lasts thirty minutes and multiplies their strength several times over. Karn estimates that even at full strength he would be only slightly stronger than Senju Hermione. Its magical versatility belongs to the temporary fused state and is not established as a permanent addition to Tsunade’s independent abilities 433.
