She is also possessive and easily unsettled by other girls’ closeness to him. Nevertheless, her jealousy does not prevent her from befriending Kushina or approving Kushina’s engagement to Lei. During a rooftop lunch, Mikoto realizes that her feelings for Lei are romantic. Their closeness quickly becomes Academy gossip. Later that day, she accompanies him to help Kushina and is horrified when a bully apparently stabs him, before his substitution reveals that he has escaped 3 4.

After the rescue, Mikoto initially resents Kushina embracing Lei. Once she learns about Kushina’s isolation and bullying, she offers to become her second friend and declares them sisters 5.

Mikoto later joins Lei, Kushina, and Hyuga Mie for lunch at Teuchi’s Noodle Shop as the group’s friendship develops 26 27.

Engagement and Fugaku’s Challenge

Yama initially refuses Lei’s request for a marriage contract with Mikoto and tells him to become stronger before seeking approval. Tension resurfaces when Lei promises another visit without consulting her. Mikoto withdraws from him on the walk home and argues with her mother. After Miko strikes and reprimands her, Mikoto reflects on her treatment of Lei and attempts to reconnect with him, only for him to collapse 18 19 20.

She follows her parents to Konoha Hospital despite injuring her legs and asks to enter the emergency room for treatment so that she can remain nearby. Once Lei recovers, he reassures and embraces her, reconciling with her 21 24.

Graduation and Border Deployment

The approaching war prompts early Academy graduation. Mikoto passes her examination and is assigned to Tsunade’s team alongside Lei and Kushina. Her initial genin work includes cleaning a bathhouse with Kushina 34 39 44.

At their graduation celebration, Lei gives Mikoto 300,000 ryō to keep for their future. When she asks whether Tsunade has displaced her as his principal wife, he explicitly reassures her that the position remains hers 45.

Mikoto later accompanies Tsunade and Lei to the worsening border situation while Kushina stays behind. After witnessing Lei eliminate the enemy shinobi encountered along the route, she remarks on how readily he kills. The group then reaches Konoha’s military camp 48 50.

At the medical ninja camp, Mikoto shares dinner with Tsunade and Lei. During the following overnight stay, Lei’s system finishes acquiring the Sasuke-version Mangekyō Sharingan from her. Yama and Miko welcome Lei, and Miko accepts Miru and Suiren after he introduces them as his women. Following a family dinner, Mikoto arranges rooms for the newcomers and privately reunites with Lei. She expresses her wish to have a child with him and plans to help the two women buy daily necessities while Lei returns to the mansion the next day 113.

Wedding Preparations and Marriage

Lei later tells Mikoto that he will marry Kushina first, while promising a grand wedding and publicly acknowledged status to every woman in his household.
