Background / History

The Sannin and her personal losses

Tsunade studied under Hiruzen Sarutobi alongside Jiraiya and Orochimaru. During the Second Shinobi World War, Hanzō of the Salamander bestowed the title of Sannin on the three teammates 7 67.

The deaths of her younger brother, Nawaki, and her lover, Dan Katō, left her with severe grief and hemophobia. Blood could make her tremble uncontrollably, and she increasingly relied on alcohol and gambling to numb herself. Her psychological condition also extinguished her interest in becoming Hokage 1 5 28 111.

Taking in Senju Chen

Tsunade’s encounter with Lin Chen interrupted this withdrawal. His vitality-rich Wood Release aura soothed her chaotic chakra and briefly eased the grief that had tormented her for years. His resemblance to Nawaki, together with the sensation of shared Senju blood, led her to regard him as hope for her nearly vanished clan 1 2.

She brought him to Konoha, refused Root’s demand to take him for examination, and sought a legitimate identity for him. Hiruzen initially registered him as a war orphan adopted by Tsunade. Following his extraordinary chakra-paper test, he officially received the name Senju Chen 4 6 8.

Wandering and returning to medicine

Tsunade eventually left Konoha. During her travels with Shizune, she moved frequently between towns and spent much of her time gambling, drinking, and sleeping. Chen later reunited with her and described his wartime achievements and reassignment to a largely powerless village post. Tsunade immediately recognized the leadership’s effort to sideline him and blamed herself for not being there to protect him 264 265.

Chen encouraged her to treat patients again. The decisive challenge was an operation to remove Exploding Insects: confronted by blood and traumatic memories, Tsunade nearly succumbed to panic, but continued with Shizune’s assistance and the memory of Chen’s encouragement. She removed every insect, emerging exhausted but with renewed purpose 266 267 269 270.

Her hemophobia initially remained a condition requiring further recovery. By the time Chen proposed bringing her back as Hokage, he explicitly confirmed to Jiraiya that it had been cured 276 472.

Rin’s emergency and the Land of Waterfalls ruins

When Chen brought the dying Rin Nohara to her, Tsunade immediately abandoned her gambling and organized emergency treatment. She identified damaged chakra pathways, fading life force, and the Three-Tails’ chakra, then began intensive treatment and surgery despite the additional danger of a curse seal on Rin’s heart 314 315.

Later, reports of a possible Wood Release power drew Tsunade to ancient ruins in the Land of Waterfalls. Her arrival inspired open relief among villagers and ninja. With broad support from Konoha’s clans and jōnin, she formally succeeded as Fifth Hokage 493 499.
