A Child's Nosebleed
"Hey-hey."
Su Ziyang chuckled as he watched Yang Tianzheng wave impatiently at him.
"You two have something to do. Are you looking for Old Xiao or Lao Jin?"
Not entirely satisfied with the answer he received from Daoist Jin, Su Ziyang explained the problem to Yang Tianzheng again.
"Oh, kid, you've come to the right person. However, given how impatient you were when I introduced you to the recipe for Chuanya Li last time, I can't easily tell you!"
Yang Tianzheng gave a very arrogant snort.
"Oh, Master Yang, just tell Ziyang, he needs it soon."
Fairy Li pleaded for Su Ziyang from the side.
Upon seeing Fairy Li plead for him, Yang Tianzheng's expression instantly softened. "Alright, you're pleading for him, I have to give you face. But I still need to teach this brat a lesson. The method I'm going to tell you is from the book 'Chuanya '. You haven't been reading carefully, have you?"
Upon hearing Yang Tianzheng's words, Su Ziyang was taken aback... then lowered his head sheepishly and said, "Please go ahead."
"It's good that you recognize your mistakes. I know it's unrealistic to expect you to master all subjects within a year, and you're already working very hard, never taking a break. But you also need to understand that you represent the culmination of our efforts. You need to work even harder."
Yang Tianzheng's words were neither too harsh nor too mild, but they were a apt warning to Su Ziyang.
The rapid progress he made in a short period of time, and the remarkable results he achieved in treating patients, made Su Ziyang somewhat proud, which is normal.
The word "master" may sound light, but it carries great weight. They are not only those who impart knowledge, teach skills, and resolve doubts, but also mentors who guide one's direction in life.
There's a saying, "After studying medicine for three years, you'll find there are no diseases that can be cured; after practicing medicine for three years, you'll find there are no effective treatments in the world."
Especially for doctors like Su Ziyang, who are general practitioners and treat all kinds of illnesses, you never know when a disease will suddenly appear that you have absolutely no idea about.
"I know, Master."
Su Ziyang nodded heavily.
"Okay. Crush some garlic and apply it to the soles of your feet; the bleeding will stop immediately. But this remedy doesn't address the root cause. You should seek treatment based on a proper diagnosis. Okay, go quickly. I have other things to do."
After telling Su Ziyang this, Yang Tianzheng didn't say anything else and turned to enter the Daoist temple.
Su Ziyang didn't say anything, and continued walking forward with his head down.
"Your master just said something to you, are you unhappy?"
Fairy Li tugged at Su Ziyang's arm and pouted, asking a question.
"No."
Su Ziyang shook his head.
"It's okay, we're already very good. Don't think too much about it, just learn slowly. They're all so old, it's normal for them to be better than you. These people care about you so much, when you're their age, you'll be even better than them. Okay, okay."
Fairy Li patted Su Ziyang on the back and pulled Su Ziyang outside.
Su Ziyang felt a warmth in his heart as he watched Fairy Li comfort him. Yes, he should keep trying.
"Hey bro, I'm here. Where are you? I'm in the parking lot downstairs."
Upon receiving the call from Liu Yanzhen, Su Ziyang and his companion quickly arrived at the parking lot.
"You're coming too?" Su Ziyang said, seeing Fairy Li still tugging at his sleeve.
"Can't I go and take a look?"
Fairy Li glared and pouted!
"Yes, yes, I can walk."
Su Ziyang quickly agreed.
"Brother, who is this?"
When Liu Yanzhen saw the two approaching, she quickly got out of the car.
Liu Yanzhen's question stumped Su Ziyang. How should he answer? Friend? Girlfriend?
Calling them friends doesn't fit their current half-arm-in-arm relationship, and calling her his girlfriend doesn't explicitly state their relationship either.
While Su Ziyang was hesitating, Fairy Li greeted him first, "Hello. I'm Ziyang's girlfriend."
"Oh my, your wife is so beautiful. Ziyang is so lucky! Get in the car, let's go take a look together. I'll treat you both to lunch."
Before Su Ziyang could react, Fairy Li pulled him to the back seat.
Liu Yanzhen drove very steadily, taking the two directly to the city hospital. Su Ziyang had been to this place before; in fact, Su Ziyang had accompanied Daoist Jin to pick up one of Liu Yanzhen's patients from here.
"Brother, let's go to my office. I'll take a look at this child's medical records first. Don't say a word. You're a college graduate, you must know the hospital's regulations."
Su Ziyang nodded without saying a word.
After lunch break, most of the doctors on duty go to eat or take a short nap.
Liu Yanzhen brought Su Ziyang to the office and showed Su Ziyang the child's medical records.
The patient is a 10-year-old boy named Xiong Guichen.
The hospital's medical records differed from the medical records Su Ziyang kept himself; both were governed by established procedures.
Admission records, various physical examination records, records of basic vital signs, daily temperature readings, doctor's orders, long-term medical orders, temporary medical orders, etc...
The child has been hospitalized here for ten days, Liu Yanzhen plans to discharge the child tomorrow.
Since all the test results were normal, there was no way to use any medication. All our department could do for this child each day was to help stop the nosebleeds.
However, stopping bleeding does not necessarily require hospitalization; it can be done directly in an ENT outpatient clinic.
Liu Yanzhen invited Su Ziyang here today to see if he could put up a fight. If Su Ziyang also fails, then tomorrow she will have to suggest that the child be transferred to another hospital or discharged.
"Brother-in-law, did you take the child's pulse? And what does his tongue look like?"
Su Ziyang looked at the medical records on the computer and found that they were almost meaningless for reference, because all the physical examinations were normal.
"I checked this child's pulse this morning. It was a weak and superficial pulse, almost imperceptible upon deep palpation. This is understandable, given the child's prolonged bleeding; weakness is to be expected."
When Liu Yanzhen spoke, Su Ziyang didn't interrupt but just nodded repeatedly.
"His tongue appearance is a bit strange. When he first came to the hospital, his tongue coating was a bit yellow, so our head doctor prescribed him a traditional Chinese medicine, Liangxue Dihuang Tang, thinking his illness was caused by blood heat. After taking it, his tongue coating was basically no longer yellow. But he's still bleeding as usual. The head doctor's idea is to transfer him to another hospital. We really don't understand it."
"Liangxue Dihuang Decoction."
Su Ziyang thought about it for a moment, and from the perspective of blood heat, this prescription could be used.
In Traditional Chinese Medicine, nosebleeds are called "niu," and this is classified as a "blood disorder."
In most cases, the main cause of nosebleeds is due to excessive heat in the blood, so most medications focus on clearing heat.
Of course, there is also a view called "Qi failing to control blood" or "Spleen failing to govern blood." In this case, there is a well-known formula called "Restore the Spleen Decoction" (Spleen-Nourishing Decoction).
"Brother-in-law, didn't you prescribe any herbal medicine?"
"Why didn't it work, buddy? To be honest, I've tried those prescriptions from the TCM internal medicine books, and I even tried Restore the Spleen Decoction. It didn't work."
Liu Yanzhen frowned deeply, clearly her confidence had suffered another blow.
"Sigh, I still want to see this child. Thinking about it like this is too abstract."
No sooner had Su Ziyang finished speaking than there was a knock on the office door.
"Physician Liu, Physician Liu! Please come quickly! My Chenchen's nose is bleeding again."
The woman spoke very urgently, and Liu Yanzhen didn't have time to talk to Su Ziyang anymore before heading towards the ward.
"Sister Zhang, please get me a long pair of tweezers and a pack of sterile gauze."
As Liu Yanzhen ran, she shouted towards the nurses' station. This child had been staying here for ten days, so the nurses' station staff were probably used to this kind of thing. They quickly went into the medicine preparation room to get the medicine.
Hearing that the child was bleeding again, Su Ziyang couldn't care less about anything else and followed the baby out of his office.
When we arrived at the ward, the child was covering his nose with a stack of tissues. The tissues were soaked through, and blood was slowly seeping onto the child's hands.
Because of the frequent occurrence of this nasal problem, the child remained very calm. He simply went to the trash can, covered his nose with one hand, and reached for a clean tissue on the bed with the other.
The child was perfectly fine and well-behaved until Liu Yanzhen appeared, at which point his expression changed instantly.
When Sister Zhang pushed the dressing cart over, the child immediately started crying loudly.
When the child cried like that, his emotional state caused his blood pressure to rise, and his nosebleed became even more profuse. Moreover, as he cried, a lot of the blood from his nose was swallowed directly into his mouth.
"Chenchen, be good, don't cry, don't cry. Let the doctor press on it, and the bleeding will stop."
The child's mother went up to hug him, but the child dodged her.
"Mom, I don't want it anymore, I don't want it anymore! I'd rather die from nosebleeds! I was wrong, Mom! Ah, Mom, I don't want it anymore! Please, Mom, don't let it stuff my nose. Please!"
The child cried out in a hoarse voice, begging for mercy, while trying to hide.
Hearing her son shout like that, the mother burst into tears. Her own flesh and blood was the one she felt the most pain for.
For a ten-year-old child to say something like "I'll bleed to death" shows how much psychological trauma this method of pressure-based hemostasis has caused to him.
It goes without saying that even a speck of dust in your nose can trigger a series of sneezes, let alone having a gauze pad shoved into your nostril, causing frequent bleeding and damage to your fragile blood vessels—that kind of pain is unbearable...
But this is the only way to temporarily stop the child's nosebleed.
When Sister Zhang and Liu Yanzhen saw the child crying so hard, their brows furrowed. They were all human, and faced with the child's cries, they felt a pang of sadness and guilt, knowing they had no better medical options.
With the child held tightly by his mother, Liu Yanzhen had no choice but to put on disposable rubber gloves and use tweezers to pick up the gauze soaked in Vaseline.
The mother, tears streaming down her face, hugged her son tightly. "Chenchen, don't cry. Mommy is so sorry, please bear with it, any more bleeding will make things worse, Mommy is so sorry..."
As soon as the child saw Liu Yanzhen walking towards them, their struggles and cries intensified, and blood splattered onto the floor and their clothes. The bright red blood dripped onto the snow-white sheets, a stark contrast...
