History

Jiraiya brings Tsunade back to Konoha at the Third Hokage’s request as the threat from Akatsuki grows. At Konoha Hospital, she treats Sasuke’s severe mental injuries from Itachi’s genjutsu. Jiraiya asks her to help protect Naruto, but she refuses because of her hemophobia 153 171.

Tsunade initially judges Kurama Yakumo too frail to continue training as a ninja. After Yakumo’s unexplained recovery and awakening of the Red Dragon Eye, Tsunade takes her as a disciple. When the eye’s strain threatens Yakumo’s life, Tsunade plans a dangerous First Hokage cell transplant using Yamato’s case as a guide. Yakumo survives; Tsunade then trains her in Superhuman Strength 283 284 290 295 296 297.

Following the Chunin Exams battle, a unanimous vote makes Tsunade the Fifth Hokage, succeeding the injured Third Hokage. She subsequently sends Naruto to train with Killer B as part of her response to Akatsuki’s pursuit of jinchūriki 865 872.

As Hokage, Tsunade directs Konoha’s preparations against Akatsuki, including tactical simulations and a call for another Five Kage Summit. Later, she joins Shinji and Research Institute personnel in planning the Ninja World’s defense and offers to secure personnel for the Bulwark Plan 946 1014 1499 1515.

Personality

Tsunade is protective of her patients and students, but does not treat their safety as a reason to ignore their choices: when Yakumo insists on searching for her missing parents, Tsunade leads the mission herself. She is also willing to acknowledge mistakes, admitting that she underestimated challengers from other villages after Yakumo’s defeat 322 780.

Her medical judgment is cautious even under pressure. She considers First Hokage cell transplantation only because Yakumo has no viable conventional treatment, and she warns Yakumo that the implanted cells could still become dangerous if her chakra falls too low 290 296.
