Her younger brother, Nawaki, and her lover, Dan Katō, both died under circumstances later attributed to schemes by Sarutobi Hiruzen and Shimura Danzō. Sanpo finds Tsunade in Tanzaku Town while she is being pursued by creditors for her gambling debts. He offers her positions as Konoha Advisor Elder and Medical Department head, while promising that Nawaki may be revived. Tsunade ultimately agrees to return with him. 62 63

After returning, she resumes work in Konoha's medical system and is seen wearing a medical-nin coat while serving as both Advisor Elder and department head. 68

Fifth Hokage

Following Uchiha Fugaku's death, Sanpo recommends Tsunade as Fifth Hokage because her appointment would stabilize the village and prevent the Uchiha from monopolizing authority. 93 She later considers stepping down to focus on research, and Orochimaru ultimately succeeds her as the Sixth Hokage. 130 132
