Background & History

Uchiha Clan Massacre Operation

Hiruzen and Danzō initially oversee the planned extermination of the Uchiha Clan. After Itachi Uchiha's Susanoo is defeated by an unknown swordsman, Hiruzen concludes that retreat is no longer possible and orders the deployment of every available force. He directs Danzō to commit Root operatives, informs Ino–Shika–Chō of his plan, and later sends the Sarutobi Clan's private force into the battle. 42 44

When Junichi Uchiha annihilates the attacking Anbu, Root, and Sarutobi forces, Hiruzen orders an immediate halt and attempts to negotiate peace. He recognizes that he cannot unite all of Konoha against the Uchiha unless they openly rebel against the village. 48

Hiruzen remains convinced that Fugaku Uchiha can restrain the clan because of Fugaku's former conciliatory stance. This assumption collapses when Fugaku reveals that Junichi has replaced him as clan head. 50 54

Root Exposure and Resignation

Hiruzen follows Fugaku and the gathered jōnin to Root's exposed base, hoping to retain control over the situation and limit the consequences of Danzō's crimes. The base contains evidence that Root abducted and experimented on Konoha ninja and clan members. 55 58

Faced with demands for Danzō's punishment, Hiruzen chooses to protect his longtime ally by immediately resigning as Hokage. He gives his robes and hat to Genma Shiranui and announces that Danzō's fate should be decided by the Fifth Hokage. 59 60

Despite resigning, Hiruzen intends to preserve influence as an elder and continue intervening in village affairs. Junichi recognizes this plan and continues dismantling Hiruzen's remaining power base. 60 61

Loss of Influence

Junichi takes custody of Danzō, reclaims Shisui's Mangekyō Sharingan, and assigns Anbu personnel to guard the Root base, effectively separating them from Hiruzen's control. 63

Hiruzen realizes that the ninja clans, civilians, and Anbu have abandoned him. He is left with little meaningful authority beyond the Sarutobi Clan and Ino–Shika–Chō. 62

He later reassures Koharu and Homura that their clans' members, embedded throughout Konoha's institutions, can restore their faction's control once a new Hokage takes office. 66

Collapse and Hospitalization

While Hiruzen is away from the village, Uchiha forces exterminate the Sarutobi, Shimura, Utatane, and Mitokado Clans. Upon learning that every Sarutobi clansman has been killed and seeing his compound consumed by flames, Hiruzen suffers a cerebral blood vessel rupture and collapses. 66 67

He awakens paralyzed and unable to speak or move freely. The damage later worsens into an irreversible, terminal condition: Tsunade diagnoses widespread brain-tissue necrosis, with dementia, respiratory dysfunction, and organ failure expected to follow. 70 80

Personality

Hiruzen presents himself as calm, reassuring, and committed to Konoha's peace, using the authority of the Hokage to rally jōnin and clans behind his decisions. He treats political legitimacy as a tool, believing that his office allows him to mobilize the village for his own strategy. 50

He is deeply pragmatic regarding Danzō. Although he recognizes Danzō's greed and dangerous ambitions, he considers those flaws useful because they make Danzō a controllable “blade” for actions unsuitable for the Hokage. 39 59

His loyalty to Danzō outweighs his public duty. Rather than allow Danzō to face immediate judgment, Hiruzen sacrifices the Hokage position to preserve his ally and retain indirect influence. 59 60

Hiruzen is also prone to underestimating Junichi and the Uchiha Clan's ability to overturn Konoha's political order. His confidence in Fugaku's obedience, the Sarutobi faction's institutional network, and his own experience repeatedly proves misplaced. 52 54 66
