Background / History

Tsunade had left Konoha and spent her time gambling on Tanzaku Street, accumulating substantial casino debt while travelling with Shizune. Uchiha Yao, Konoha’s Fifth Hokage, located her and sought her return for her medical expertise. 161 162

She initially refused to return, retaining resentment toward Konoha after its leadership had rejected her wartime proposal to assign medical ninja to every genin team. Her hemophobia was also a major barrier to resuming medical work. 162 167

After returning to Konoha, Tsunade accepted a potion offered by Yao. Testing herself against the sight of his blood confirmed that her hemophobia had been cured. Yao then appointed her head of the Medical Department and tasked her with reorganizing Konoha Hospital and training medical ninja. 167
