Background
Makoto entered Shin'ō Academy from Rukongai. Though mediocre in conventional combat disciplines—including Hakuda, Kidō, Zanjutsu, and Shunpo—he excelled in Kaidō and medical skills, and had already attained Shikai. He commonly partnered with Madarame Ikkaku and Ayasegawa Yumichika during his academy years. 2 12
Concerned with survival in the dangerous Soul Society, he chose the low-casualty Fourth Division as a practical path while pursuing his genuine interest in medicine and the unusual structure of spirit bodies. His outlook was shaped by a strong sense of crisis, loneliness, and a refusal to accept that nobles or rulers were inherently superior. 7 13 18 102
Unohana Retsu recognized him as a Kaidō prodigy and accepted him as her disciple. The relationship gave Makoto both protection and access to advanced medical knowledge, though it also placed him under greater attention within Seireitei. 12 13
After decades as Unohana’s disciple and the Fourth Division’s Vice-Captain, Makoto became widely respected as a physician and administrator. He was awarded Intermediate Noble status after assisting the Sixth Division in the Human World, but continued living in the Kuchiki estate rather than his own Chiba-family courtyard. 197 202
Personality
Makoto ordinarily presents himself as gentle, modest, professional, and considerate. He consistently treats injured Shinigami regardless of personal feelings, maintains boundaries with flirtatious patients, and earns loyalty through medical ability and reliability. 90 142 155
Behind this demeanor, he is deeply cautious and calculating. He gathers intelligence, conceals the scope of his powers, manipulates perceptions when necessary, and treats power as indispensable to survival. His long-term objective becomes the reform of Soul Society and the removal of forces loyal to the Soul King Palace. 78 229 419 552
He is protective of people he considers his own—especially Unohana, his patients, allies, and Soul Society itself—but this protection is intertwined with a possessive belief that his territory must not be destroyed by outsiders. 119 423 552
Appearance
Makoto is described as a fair-skinned, delicate-featured young man whose usual gentle expression makes him appear harmless. In his Shinigami identity, he generally wears a standard uniform rather than displaying his noble crest. 202 441
- Wears glasses on several occasions. 118 212
- His Bankai mental-space form develops a dark haori, lengthened pale hair resembling a crown, and ink-like markings across his body. 333
- Can alter his outward form through Kishi, including creating a human-like shell for his Kurosaki Makoto identity. 343
Abilities & Skills
Kaidō and Medical Skills
Makoto is a medical prodigy whose treatment ability surpasses that of ordinary seated Fourth Division officers.
- Personally treated more than forty patients during a single overnight crisis. 142
- Can treat severe spirit-body injuries, including captain-class patients such as Zaraki Kenpachi. 142
- Developed Spirit Body Layering to strengthen spirit-body structure, spiritual-particle density, and Reiatsu tolerance. 229 255
- Released All Things in Nature to heal injured Shinigami throughout Seireitei. 701
- Uses healing as both protection and a means of establishing influence over the wounded. 706
Seed of the Forest
A special Reishi construct that supports Makoto’s growth and surveillance.
- Absorbs scattered Reiryoku to replenish his Reiatsu. 65
- Can be integrated into Shinigami and Hollowfied Shinigami, increasing his absorption rate. 156
- Allows him to monitor loyalty when planted in injured Shinigami. 706
- Production is limited, restricting the speed at which he can accumulate Reiryoku. 78
Reishi Control
Makoto manipulates surrounding Reishi on a vast scale rather than depending solely on his own spiritual reserves.
- Alters terrain with grass, trees, roots, wooden constructs, and massive palms. 76 825
- Creates barriers capable of withstanding repeated Hadō bombardment. 519
- Forms Reishi Threads, arrows, and dual Reishi Blades. 362 471
- Can detect Aizen through the Reishi associated with Kyōka Suigetsu. 237
- Cannot directly interfere with the Reishi constituting another person’s spirit body. 369
All Things in Nature
Makoto’s Zanpakutō, initially presented as a healing-type Shikai, possesses far greater offensive, defensive, and spiritual functions.
- Produces wood-type abilities and massive natural constructs. 76 263
- Its spirit can materialize independently and was temporarily separated from Makoto by Muramasa. 209 224
- Its apparent ability can be imitated or exploited, causing others to associate the technique with Makoto even in his absence. 514 551
- Its different Bankai manifestation, Imperial Decree of All Things, reveals the deeper authority of the Zanpakutō. 333
Authority of Name and Kishi
After entering Imperial Decree of All Things, Makoto gains authority beyond ordinary Shinigami, Hollow, and Quincy power.
- Can assign his Name to blank Names and extend his will through Reishi. 334
- Marked Reishi across Soul Society and Hueco Mundo. 334
- Learns to manipulate Kishi as part of “All Things.” 340
- Uses Kishi to create human disguises and repair otherwise extraordinary injuries. 343 378
- Reaches an evolutionary state that transcends Shinigami and Hollow boundaries. 340
Quincy Techniques
While acting as Kurosaki Makoto, he uses Quincy abilities without exposing his Shinigami identity.
- Identifies himself as a pure-blood Quincy searching for his lineage. 345
- Learns Ishida-family combat techniques and Quincy history from Ishida Ryūken. 351
- Uses Reishi Arrows powerful enough to destroy Kurotsuchi Mayuri’s Bankai. 364
- Possesses Blut Vene at a level compared to the Quincy progenitor. 471
- Can form Reishi Blades modeled after Quincy Cross Blades and duel Yamamoto in close combat. 471

Trivia

- He is the subject of the “Special Exchange Association for the Conquest of Doctor Chiba” and later the Pro-Makoto Association. 90 229
- Despite his public image as a healer, his earliest strategic plan was to layer hidden abilities—wood-type powers, Limit Spirit Body, and Bankai—as successive safeguards. 78
- He views medicine alone as insufficient to correct Soul Society’s injustice: “practicing medicine cannot save Soul Society.” 419
