Background

Shikotsumyaku is the Kaguya clan's Kekkei Genkai. Konoha's research into the Senju, Uchiha, Hyuga, and Uzumaki bloodlines found that it shared a subtle connection with all four clans, while the Kaguya and Hyuga clans were said to have maintained an ancient relationship. 53 57

In Kimimaro's case, the bloodline is afflicted by a severe hereditary defect. Its activation causes constant marrow-deep pain and a “hunger” in his bones; without filling that emptiness, Shikotsumyaku becomes fatal rather than purely empowering. Orochimaru prolongs Kimimaro's life through medicine, while Kimimaro endures the condition through willpower. 55 56

When Kimimaro's bone spear drains Naruto's blood, the resulting reaction radically alters him. The transformation apparently cures his bloodline illness, changes the behavior of his bones, and coincides with the awakening of Byakugan-like and later Tenseigan-class ocular powers. 56 57 137

Shikotsumyaku allows its wielder to grow, shape, and project bones from their body at will.

Blood Absorption and Transformation

Kimimaro's bone spear displays an abnormal blood-draining reaction after piercing Naruto's hand.
