Background

Before Konoha had a formal medical system, Tsunade maintained the Senju ancestral home’s best available treatment facilities and personally treated injured shinobi. Her healing chakra rapidly mended wounds, bruises, and muscular fatigue, making her care notably comfortable for patients. 29

She organized scattered medical knowledge and techniques into a systematic discipline, with the goal of cultivating medical ninja on a large scale. Tsunade documented Uchiha Gen’s training results as practical data for refining this system; proving its feasibility would help convince Konoha’s leadership to support it. 40

During the war, Tsunade established a temporary battlefield medical area, selected more than a dozen Genin with medical aptitude, and trained them as the first prospective medical-ninja assistants. Her medical capacity remained limited by the lack of an established system, so she personally prioritized severely injured shinobi and key combatants. 81 83

She later worked with Orochimaru to secure support for establishing a village Medical Department. Although the initial proposal was rejected during wartime, Tsunade, Orochimaru, and supporting jonin continued pressing the issue until Hiruzen Sarutobi approved a limited trial plan. 82 144 145

Tsunade is Hashirama Senju’s granddaughter and, after Nawaki’s death, his only surviving descendant. Nawaki had been Orochimaru’s student, making the sight of Orochimaru taking on a new team emotionally complicated for her. After Mito Uzumaki’s death, Tsunade became the Senju Clan’s representative in the contemporary ninja world. Personality

Tsunade is magnanimous and largely free of the Senju–Uchiha clan prejudice. She treats Uchiha Gen as a fellow shinobi and Orochimaru’s disciple rather than as a member of a rival clan, openly sharing her medical knowledge when she believes it should be taught. 40

She is demanding but nurturing as an instructor. Tsunade tests students through practical work, observes their weaknesses closely, and challenges their confidence—yet she readily recognizes exceptional talent and supports those who prove themselves. 39 81 82

Her ambition is tied to her grandfather’s legacy: she wants medical ninja to change warfare by reducing preventable casualties and improving Konoha’s long-term fighting capacity. 40

Despite her expertise as a shinobi and physician, she initially lacks political experience. Before her first formal proposal at the Jonin Conference, she assumes that presenting the medical system and receiving her allies’ votes will suffice, without organizing support in advance. Uchiha Gen advises her to contact sympathetic jonin and use existing connections, after which she leaves the political preparations to Orochimaru. 128

She places the medical system above concerns about the Senju Clan’s declining status. When Gen warns that an unsuccessful proposal could undermine perceptions of Senju influence, Tsunade dismisses this as secondary to advancing medical training. 128

Early Medical Training

Tsunade treats Orochimaru’s team at the Senju ancestral home and becomes the first major source of systematic medical-ninjutsu instruction for Uchiha Gen. She teaches through theory examinations, live demonstrations, and supervised practice with the Hemostasis Technique. 29 39 40

Her first appearance occurs when Orochimaru and Jiraiya bring the young ninja to visit Mito. At Mito’s request, Tsunade takes the team for treatment and examination and is instructed to provide each of them with a collection of Fuinjutsu teaching materials. 28

Land of Rain Medical Area

During the war, Tsunade operates a temporary medical zone, trains early medical assistants, and uses Uchiha Gen as both a treatment assistant and a test case for advanced instruction. She offers him B-rank missions in exchange for helping her and learning the Mystical Palm Technique. 81 83

The heavy flow of wounded exposes the limits of relying on a few capable healers. Tsunade personally treats severe cases and important combatants while developing a larger support staff; Gen later becomes capable of taking charge when senior medical personnel are absent. 81 89 91

Medical-Ninja System Initiative

The agreement is broader than the original student wager. Orochimaru’s commitment to establishing the system is not contingent on whether Gen masters one technique within the allotted time; Gen’s success instead confirms that his medical talent deserves continued training under Tsunade. 84

Battles Against Chiyo

Tsunade fights alongside Orochimaru against Chiyo during the war. Later, when Chiyo ambushes Uchiha Gen with the Ten Puppets of Chikamatsu, Tsunade intervenes with Painful Heavenly Foot while Orochimaru attacks with snakes. 95 119

Her intervention breaks the ground beneath the puppets’ Three Treasures Absorption formation, forcing Chiyo to abandon its suction attack. At the same time, Tsunade’s split-off slugs provide healing support to wounded Konoha ninja. 119

Establishment of the Medical Department

Tsunade proposes a Medical Department before the Jonin Council. The wartime proposal initially fails, but continued support from Orochimaru, the Uchiha jonin, and others leads Hiruzen Sarutobi to approve a limited initial plan. 144 145

Before the conference, Gen identifies her lack of political preparation and advises her to coordinate with Orochimaru. He also promotes the initiative within the Uchiha Clan, emphasizing how the battlefield medical area reduced casualties and improved sustained combat capability. 128 141

Hiruzen authorizes trial training rather than an unrestricted rollout, limiting village funding to thirty million ryo. Tsunade must obtain further funding herself if necessary. The Uchiha subsequently cooperate with the initiative and arrange for Nonō to teach basic medical skills to selected clansmen. 145 146

The new Medical Department also attracts interest from other village factions. Danzō reserves several training places for Root, although the trainees have not yet become capable medical personnel when he later considers recruiting established practitioners. 180

After Gen’s Second Body Modification and successful attempt to contract the Third Rashomon Gate, Tsunade examines his injuries and helps him recover.
