Background / History
Tsunade graduated from Konoha's Ninja Academy as one of the exceptional shinobi produced during Sarutobi Hiruzen's administration. She later became a disciple of the Third Hokage and one of the Sannin. 1 39
During the Second Shinobi World War, her younger brother Nawaki was killed by an explosive-tag trap. Dan Katō also died during the war. These losses left Tsunade with hemophobia: seeing or touching blood causes her to shake and lose strength. 34 42
After the war, she became despondent and drifted among gambling dens rather than returning to Konoha. Her absence became strategically dangerous as tensions between villages grew, prompting Hiruzen to dispatch Minato's squad to retrieve or protect her. 34
Appearance
Tsunade is a blonde woman with an imposing, mature presence that contrasts with her disheveled gambling habits. 35 37
- Long golden hair, commonly tied into two low ponytails. 37
- Exquisite facial features and a notably curvaceous build. 35 37
- Wears a grey undershirt beneath a sky-blue coat; the coat does not bear the later “Gambling” kanji. 37
- Her gambling-table appearance may include tired eyes, a snack in her mouth, and a bottle of sake at hand. 37
- A poster depicts her in a standard Konoha jonin vest, with windblown hair and a murderous gaze. 35
Personality
Tsunade is proud, boisterous, and fully aware of her reputation. When confronted about her debt, she grandly identifies herself as the granddaughter of the God of Shinobi, the Senju clan's sole heir, Konoha's strongest taijutsu user, and the world's finest medical ninja. 39
Her confidence is undermined by a reckless gambling habit. She plays impulsively, disregards sound poker strategy, and has accumulated debts exceeding ten million yen. Even so, she shows flashes of sharp judgment, such as recognizing the possibility that an opponent is bluffing. 37 38 39
Tsunade is also opportunistic and direct. Once Mai demonstrates profitable Texas Hold'em skill, she quickly proposes an exchange: she will teach him taijutsu if he teaches her poker. 41
Abilities & Skills
Taijutsu
Tsunade is regarded as Konoha's strongest taijutsu expert and has sufficient prestige that Might Duy considers her an idol. 34 35 39
- Her Heavenly Foot of Pain is a downward chakra-enhanced strike capable of devastating an area. 25 41
- Mai models his Outer Style: Bombing Axe after the movement principles of Tsunade's Heavenly Foot of Pain and the Raikage's Heavy Thunder Axe. 25
- She agrees to provide Mai with taijutsu guidance after returning to Konoha. 41 48
Medical Ninjutsu
Tsunade is recognized as the world's foremost medical ninja and is credited with pushing for the medical-ninja policy and founding Konoha's Medical Corps during the Second Shinobi World War. 39 41
- Her medical policy greatly reduced Konoha's wartime casualties and brought medical ninjas into the village's mainstream structure. 41
- She believes Mai has the aptitude to study medical ninjutsu and potentially inherit her mantle, though he declines to prioritize it. 41
- She can manipulate chakra into water for the group to wash themselves after the Kumogakure ambush. 48
Gambling
Despite styling herself as a skilled gambler, Tsunade's play is frequently reckless and technically poor. 37 38
- She calls large bets with weak hands and fails to apply basic positional or betting discipline in Texas Hold'em. 37 38
- Her poor results have left her heavily indebted and unable to sustain her gambling without borrowed money. 36 39
- Under Mai's guidance, she and Mai win enough money for her to accept their negotiated return to Konoha. 39 41
Hemophobia
Tsunade's psychological trauma is her most severe documented limitation. It emerged after the Second Shinobi World War and is tied to the deaths of Nawaki and Dan Katō. 34 42
- Seeing or touching blood makes her tremble and become physically weak. 42
- During the Kumogakure ambush, Minato's killing of an attacker triggers her paralysis and leaves her exposed to concentrated ninjutsu fire. 41 42
- Mai uses Gongshoudao to pull her clear of the attack, while Minato later realizes why Hiruzen had insisted on her protection. 42
- She asks Minato and the team not to reveal her condition. 42
- By the time Minato returns from pursuing the attackers, she has regained enough control to play poker with Kakashi, Guy, and Takumi. 47

Story Role / Major Arcs
Retrieval Mission
Team Minato receives an A-rank mission to retrieve Tsunade from Tanzaku Town or remain at her side to protect her. Hiruzen needs her return both for Konoha's security and for the deterrent value of her reputation. 34
The team finds her gambling at Tanzaku Town's largest casino, where she is losing heavily. Tsunade refuses Minato's initial request to return and challenges the squad to capture her if they can. 36 37 38
Poker Agreement
Mai exposes Tsunade's debt and negotiates a condition: if she can win ten million ryō in ten days, she will return with the team. Minato lends her 500,000 ryō, secured by her coat, while Mai and Tsunade agree to divide winnings seventy-thirty in her favor. 39
After the gambling venture produces a profit, Tsunade accepts Mai's proposed exchange of taijutsu instruction for Texas Hold'em lessons and agrees to return to Konoha. 41
Kumogakure Ambush
On the return journey, a Kumogakure squad ambushes the group to target Tsunade. Minato kills an attacker, but the sight of blood triggers Tsunade's hemophobia and leaves her unable to defend herself. 41 42
Mai rescues her from the concentrated attack, and Minato later pursues the retreating enemies to prevent them from exploiting knowledge of her condition. After the squad is eliminated, Tsunade returns to Konoha with Team Minato. 42 47 48
