Big Mom founded Totto Land as a country where different races could live together. Residents could live without worrying about food or clothing, but they paid a portion of their lifespan to remain there. Leaving meant surrendering all their remaining lifespan 475 625.

- Soul-powered inhabitants — Big Mom granted souls to plants, animals, furniture, and other objects. Homies also served in her armed forces and maintained public order 475 606 644.

History
Big Mom had established Totto Land while expanding through the New World. Reg later sent Fitch into the Big Mom Pirates as a chef; Fitch became an aide to Charlotte Perospero and relayed information from within the kingdom 136 165 267. The Totto Land waters were also the site of a battle involving the Roger and Golden Lion Pirates 165 174.
After Katakuri’s capture, Big Mom arranged a ransom exchange in her own waters. Reg arrived with allies from Elbaf; Big Mom attacked, but the encounter ended in a negotiated withdrawal 287 292 302 304. A later marriage arrangement tied Custard of the Charlotte Family to Berg of the Kingdom of Zh, alongside proposed trade and nonaggression terms 509 513.
Big Mom subsequently declared war on the Kingdom of Zh, triggering alarms and mobilization throughout Totto Land 599. Her fleet suffered heavy losses in the Apple Sea, while fighting spread aboard the opposing Floating Fortress 605 606. Mother Carmel then turned the kingdom’s Soul Messengers against its residents, harvesting lifespans across the islands. Katakuri chose to fight the Soul Messengers and protect his siblings 644 645.
As Big Mom’s defeat approached, Katakuri offered all of Totto Land’s wealth in negotiations with Reg, seeking protection for his family and a future for Custard 650.
