Background

Tsunade is the daughter of Hashirama Senju and Mito Uzumaki. She and Dan Katō had a daughter, Kushina Senju, whose death was faked during wartime to prevent a marriage decree from being enforced; Kushina subsequently lived under the name Kushina Uzumaki. Naruto is Tsunade's grandson through Kushina. 18 42 244

She lost Dan and her younger brother Nawaki during the war. After receiving news of the Nine-Tails attack, she fled Konoha without knowing that Kushina had borne a son. By the time Naruto learned of his Senju heritage, Tsunade had spent years away from the village, drinking and gambling while unable to face her losses. 18 47

Orochimaru located Tsunade and revealed that Naruto was alive. He offered to restore Kushina, Dan, and Nawaki if Tsunade healed his damaged arms, but admitted that he intended to destroy Konoha afterward.
