Background
Unohana was a captain of the original Gotei 13 and held the title of First Kenpachi roughly a thousand years before Wang Xun’s service. Her past in Rukongai and the 11th Division was defined by killing and swordsmanship, in sharp contrast to her later role as a healer 56 79 84 125.
An unnamed swordsman from Zaraki District once wounded and defeated her in Kendō. After that encounter, she found no opponent who could rekindle her interest in battle. She gradually restrained her killing intent, put aside her killing blade, and devoted herself to Kaidō, eventually becoming Fourth Division Captain 133 241.
Unohana learned Kaidō from Zero Division member Tenjirō Kirinji. She recognizes his extraordinary medical skill and strength, but bluntly describes him as arrogant 134.
Appearance
- Usual clothing: A plain Shihakusho beneath a captain’s haori, with an understated, elegant presentation 79 163.
- Hair: Long hair normally arranged over her chest; during sword training, she sometimes leaves it loose over her shoulders 123 189.
- Expression and bearing: A gentle smile, soft voice, and composed posture that give her a reassuring presence 56 79.
- In battle: Her customary warmth can give way to an eerie, delighted smile as her suppressed fighting instinct emerges 132 133.
Personality
Unohana is patient, observant, and quietly authoritative. When she catches Wang Xun wandering through hospital wards at night, she questions him without raising her voice, examines his claimed interest in Kaidō, and offers him a legitimate route to further instruction. Her gentleness does not prevent her scrutiny from placing him under intense pressure 56.
She takes considerable pride in nurturing talent. Initially impressed by Wang Xun’s healing, she later becomes fascinated by his potential as a swordsman and hopes to train him into an opponent capable of fighting her without restraint. Their sparring reawakens her pleasure in battle, occasionally making it difficult for her to control her bloodlust 99 133 181 183.
Unohana is also politically cautious rather than passive. She resents noble interference in the Fourth Division, but recognizes that her position requires her to consider Yamamoto’s wishes and the balance between the Gotei 13 and the nobility. She seeks mutual understanding with Yoruichi instead of openly attacking the established order 84 109.
Abilities & Skills
Kaidō and Medicine
Unohana’s healing combines extremely precise Reiryoku control with extensive medical knowledge. During emergency treatment, she directs threadlike Reiatsu into separate internal injuries simultaneously 94.
- Treats injuries ranging from ordinary wounds to damaged spiritual organs and souls 119.
- Personally removes a compound toxin affecting Soi Fon’s nerves and spiritual body 79.
- Identifies that part of Rangiku Matsumoto’s soul has been stripped away and prescribes special herbs and a barrier for a recovery expected to take months or longer 151.
- Heals her own shallow wounds almost instantly, including while fighting Wang Xun 133 233.
- Acknowledges that even she cannot easily master Wang Xun’s newly developed Double Kaidō treatment 120.
Yachiru Swordsmanship
Unohana’s personal sword style integrates Kendō, killing intent, and Reiatsu control. She teaches that sword techniques ultimately serve the purpose of killing an opponent, rather than existing as empty forms 123 132.
- Uses killing intent to teach Wang Xun to withstand danger instead of instinctively retreating 123.
- Encourages ruthless, unpredictable attacks rather than applying a healer’s insistence on precision to every movement 126.
- Slays an Adjuchas with a single stroke during the temporary camp’s defense 125.
- Cuts apart Wang Xun’s miniature Black Coffin, other Hadō attacks, and later his Cero with her blade 182 183 233.
- Possesses swordsmanship that Aizen considers beyond the reach of the vast majority of captains, even without releasing her Zanpakuto 233.
Kidō and Dual Incantation
Unohana ranks among the Gotei 13’s most accomplished Kidō users, with particular expertise in Kaidō and Bakudō 93.
- Demonstrates two masses of Reiatsu operating at different frequencies without interfering with one another 93.
- Explains that Dual Incantation depends on allowing two Kidō to coexist, not forcibly fusing them 93.
- Personally teaches Wang Xun the technique and offers guidance alongside his planned studies with the Kidō Corps 93 99.
Reiatsu, Movement, and Perception
- Reiatsu: Can shift from a gentle healing presence to a cold, blood-soaked pressure. During sparring, her increasing output threatens to attract attention from outside the training grounds 181 182.
- Shunpo: Closes distance rapidly enough to overwhelm Wang Xun’s reactions and move between his combined sword and Kidō attacks 182 183 189.
- Perception: Detects something unusual near Aizen’s concealed position when arriving at the ruins 233.
- Forensic knowledge: Recognizes a concealed fatal puncture as consistent with a Quincy Reishi Arrow 226 227.
Zanpakuto
Minazuki
- Type: Biological-type Zanpakuto 133.
- Shikai: Transforms into an enormous, flat creature that heals injuries by taking a patient into its mouth. Unohana uses it to swallow and then spit out Wang Xun, leaving his sparring wounds healed but his body covered in slime 133.
- Combat limitation: Unohana considers its Shikai unsuitable for combat 233.
- Bankai: Available to her, but not released in the supplied events. She considers using it when ordinary methods prove insufficient to decisively defeat Wang Xun’s stronger Hollowfied form 233.
Story Role / Major Arcs
Recruiting Wang Xun
Unohana first encounters Wang Xun after discovering his nighttime visits to Fourth Division wards. His demonstration of Kaidō earns her attention, and she invites him to seek instruction openly or consider joining her division. After examining the emergency treatment he performed on Soi Fon, she offers systematic medical training and personal instruction 55 56 79.
Once Kiyōnosuke Yamada has taught Wang Xun everything he can, Unohana begins supervising his education during treatment duties. His rapid progress convinces her that he might eventually surpass her in Kaidō 93 99.
She subsequently asks Yoruichi to approve a transfer and offers Wang Xun a future lieutenant’s position. On joining the Fourth Division, he becomes Fourth Seat and her personal attendant. Unohana assigns him a difficult spiritual-channel injury under her observation; his successful treatment dispels younger members’ doubts about his appointment 109 110 118.
Noble Interference and the Medical Institute
Unohana uses her growing connection with Wang Xun to build an understanding with Yoruichi. Both captains are dissatisfied with noble interference, particularly the Shin’ō Medical Institute’s expansion and the planned transfer of Yamada out of the Fourth Division 84 109.
At a flower-arranging gathering, she raises the mass disappearances of Shinigami and Rukongai souls and hints at a connection to the institute. Later, she warns Wang Xun that representatives of the institute and Central 46 will inspect the Fourth Division, a visit he associates with Tokinada Tsunayashiro’s campaign against him 109 181.
Awakening Her Fighting Instinct
During a rescue operation, Unohana arrives to defeat attacking Hollows and criticizes Wang Xun’s clumsy Kendō. She agrees to teach him after he says he wants the strength to protect those behind him 121 123.
Their lessons advance from wooden swords to real blades. When Wang Xun grazes her arm, the wound awakens her long-suppressed delight in battle. She resolves to teach him the deeper substance of Yachiru Swordsmanship and begins imagining him as a future opponent—and potentially a future Kenpachi 132 133 136.
The pair later move their training beneath Sōkyoku Hill, where Unohana pushes him to reveal his hidden strength. His Hollowfication surprises but excites her. One bout ends with her blade piercing his chest after his power is exhausted; subsequent sessions show her taking greater care to keep him alive while still driving him to his limits 182 183 189.
Lieutenant Appointment and Quincy Evidence
Following Yamada’s transfer to the Shin’ō Medical Institute, Unohana appoints Wang Xun lieutenant. She emphasizes that the Fourth Division’s medical role is indispensable because only survivors can continue fighting 220.
Together, they examine the murdered Fifth Division Third Seat and identify evidence of a Reishi Arrow concealed beneath an apparent Hollow wound. Unohana reports the findings to Yamamoto while acknowledging that Quincy methods might also have been used to mislead investigators 226 227.
Hollowfication Incident and Detention
Urahara alerts Unohana that the Wang Xun accompanying her is an impostor. She subsequently confronts the real Wang Xun near the fallen captains and fights him when he refuses to explain himself. His enhanced Hollowfication, regeneration, and stamina bring the battle to a point where she considers Bankai 231 233.
Afterward, she treats his injuries and learns about Aizen’s experiments and Kyōka Suigetsu. At Central 46’s hearing, Wang Xun’s testimony supports the case against Urahara and Tessai, and Unohana is ordered detained. She initially prepares to resist, but relinquishes her sword after Wang Xun confirms his identity through private memories of their training and urges her not to fight 234 235 236.
Reinstatement and Later Sparring
Yamamoto personally releases Unohana from the Maggot’s Nest as war with the Quincy approaches. He informs her that Wang Xun has defeated Kiganjō and become Kenpachi, praising the student she trained. She resumes command of the Fourth Division, while Wang Xun recommends Isane Kotetsu as a potential lieutenant 242.
Unohana later watches Kirinji’s dangerous test of Wang Xun and checks on his recovery afterward. Following his treatment in Kirinji’s healing pools, Wang Xun nearly defeats her in a sword-only spar, bringing his blade to her throat. She recognizes how close he has come to surpassing her and ends the bout after he notices her delighted expression 258 259.
