The Mind Flayers, also known as Illithids, are extraplanar beings associated with psionics, hive minds, and the conversion of intelligent creatures into new Mind Flayers. Their population is expanded through tadpoles that parasitize a host’s brain and eventually replace it. Most transformed individuals lose their former personalities, although memories from the host life can influence the resulting Mind Flayer 1.

One Mind Flayer operated a transformation facility containing stasis pods and a pool of tadpoles. It immobilized captives with psionics and implanted tadpoles into their eyes, intending to convert them into new Mind Flayers 1. Karl Ares survived this process because his incompatible memories from another world overwhelmed the tadpole, causing it to enter a dormant state before eventually dying 1 5 6.

A separate Mind Flayer emerged from the conversion of a Hobgoblin in a dungeon base. Brain Eaters created it as part of a plan to expand the local Mind Flayer population and eventually restore their dead master’s consciousness through a colossal brain 36. Dungeon Base and Goblin Conflict

A Hobgoblin is later converted into a new Mind Flayer by Brain Eaters. The newborn creature retains its former memories but becomes dominated by hunger and the need for intelligent brains 36 38. It establishes control over a forest base and begins hunting nearby goblins.
