Background

During the Third Shinobi World War, Konoha faces severe personnel and supply shortages. Four months into the conflict, Hiruzen Sarutobi orders layoffs at Konoha Hospital and other departments so resources can be redirected to shinobi on the front lines. The decision leaves civilian workers, including errand staff, without reliable income while food and medicinal-ingredient prices rise in the village 1.

Konoha repeatedly deploys medical personnel to the eastern coast of the Land of Fire. One such force, led by Shinku Yuhi, includes more than a hundred medical ninja and is sent into the Death Forest battlefield despite the lack of supplies and the high risk faced by non-combatants 5 6.

The village remains under pressure after the coastal campaign. Its defensive forces at the Valley of the End lack basic equipment, and Konoha fears a larger offensive by the allied forces of four major villages 33. Even so, its leadership continues to maintain regular village operations, including the Ninja Academy, routine missions, ANBU assignments, and the Chunin Exams 34 38 41.

Government and Institutions

Hokage Administration

Hiruzen Sarutobi, the Third Hokage, directs Konoha’s wartime policy. He authorizes hospital layoffs, assigns battlefield reinforcements, sponsors Qian Yun’s academy admission, and later places him in ANBU rather than allowing other leaders to recruit him 1 34 39.

Koharu Utatane and Homura Mitokado participate in discussions concerning clan unrest, Root casualties, compensation, and Orochimaru’s research funding 1. Konoha Hospital

Konoha Hospital provides medical treatment and initially employs civilian errand workers. Wartime shortages lead to layoffs, while experienced medical ninja are sent toward active fronts 1 5.

Internal Politics

Konoha’s wartime leadership is marked by distrust between institutions and clans. Its advisors worry that layoffs could provoke resentment among the Hyuga and Uchiha Clans, while Root’s losses and lack of compensation become a separate concern 1.

The Uchiha Clan is monitored by both Hiruzen and Danzo’s forces. During one surveillance assignment, Qian Yun discovers that the Third Mizukage attempts to recruit the Uchiha Clan to relocate to Hidden Mist territory 39.

Konoha’s upper ranks are also accused of withholding battlefield supplies. After Qian Yun turns in a large cache of equipment, Shinku Yuhi keeps it within the frontline forces because sending it back to the village would likely result in most of it being retained there 33.

Military Operations

Eastern Coast Campaign

Konoha dispatches a large medical-ninja team to the eastern coast, where it is ambushed by the Seven Ninja Swordsmen of the Mist. The Uchiha vice-captains are killed, while Shinku Yuhi continues fighting despite illness or injury 5 6.

The Death Forest camp becomes a major treatment site for severely wounded Konoha shinobi. Medical personnel work in critical care while the camp faces enemy raids and infiltration 7.

Conflict with Hidden Sand

Konoha forces later fight Hidden Sand ninja along the coast. Minato Namikaze’s intervention allows Konoha to turn the battle in its favor, but Hiruzen orders the forces not to completely destroy Hidden Sand in order to preserve the wider balance of power 14.

Kushina Uzumaki and other Konoha medical ninja are targeted during the conflict. Konoha’s forces suffer ambushes while evacuating wounded personnel, and Kushina’s Nine-Tails chakra emerges during one attack 15.

Valley of the End Defense

Konoha’s Valley of the End forces prepare for an expected assault by the four major villages. The defensive line is short of weapons, with some incoming genin forced to sharpen branches into spears 33.

Qian Yun later returns to Konoha through the Valley of the End after being pursued by shinobi from several villages. Kushina welcomes him back, and Shinku Yuhi identifies him as the medical ninja previously lost from the southern coastline battlefield 32 33.
