Initially suspicious and resistant, Tsunade allows her assistant Shizune to take Sōji's case under strict conditions 6. Over the course of a year, she indirectly guides his medical treatment, ultimately curing his Shikotsumyaku-related illness 7. Afterward, Sōji joins her travels across the nations as a merchant guard, during which Tsunade teaches him basic medical principles, the Chakra Enhanced Strength technique, and eventually her ultimate signature abilities—the Yin Seal and Creation Rebirth 8 12 13 34.

Tsunade sends Sōji a coded invitation to meet in Tanzaku Town, concealed in an official Konoha letter. He arranges to visit the Land of Fire under the pretext of trade negotiations 151. Their reunion begins with Tsunade attacking him with Chakra Enhanced Strength punches, which he deflects without retaliating. During their conversation, Sōji identifies her hemophobia as completely healed and insists that they acknowledge their feelings as equals rather than as teacher and former follower. He argues that she has never truly let go of Konoha, predicts that she will become Hokage, and proposes that their eventual leadership of Konoha and Kirigakure could secure peace through an alliance 169. After learning that the First's necklace helped awaken Sōji's Wood Release, Tsunade recalls a complete manual compiled by Tobirama from Hashirama's techniques and notes. It is stored behind a barrier in the Senju Old Residence, with access known to a summoning slug left by Hashirama. Although originally intended for a Konoha ninja who awakened Wood Release, she chooses to give it to Sōji 170.

Tsunade temporarily accompanies Sōji, Shizune, and the Land of Water delegation to Konoha and retrieves the manual. Before leaving, she personally delivers it in a Senju bloodline-sealed wrapping and urges Sōji to study Senjutsu with the Slug Sage of Shikkotsu Forest.
