Background

Shizune travels with Tsunade as her disciple, medical assistant, financial caretaker, and the person most often responsible for managing the consequences of Tsunade’s gambling and drinking. She is also Tonton’s caretaker, using the pig’s tracking ability to locate Tsunade when she disappears into gambling houses. 8 10

When Yamada Sōji seeks treatment for his terminal bloodline disease, Shizune is immediately more sympathetic than Tsunade. She privately meets with him at Yamada Pharmacy, examines his pain-relief ointment, and presents his payment offer and trauma-treatment theories to Tsunade. Her desire to take on the difficult case persuades Tsunade to allow the treatment under supervision. 5 6

Over the following year, Shizune helps develop and administer Sōji’s treatment. After his recovery, she gives him complete records, medicine formulas, dosage guidance, and relapse-prevention instructions. 7

During the subsequent journey, Shizune becomes one of Sōji’s instructors in medical ninjutsu, providing medical notes and chakra-pathway diagrams. She later assists in Karai’s operation, manages his postoperative care, and independently removes Explosive Insects from Amai. 12 25 27 31

After Sōji departs from Tanzaku Town, Shizune catches up with Tsunade at the fork in the road and notices Tsunade’s unusual smile and lingering reaction to his farewell. 36

By Sōji’s later return to Tanzaku Town, Shizune has known Tsunade for over a decade. She witnesses an unfamiliar degree of ease and intimacy between her mentor and Sōji, then accompanies them and the Land of Water delegation to Konoha. 169 170 171

Personality

Shizune is compassionate and professionally driven. She sees Sōji’s fear of his body deteriorating and wants to treat him despite the political risks attached to his Kaguya heritage. She is also practical, diligent, and notably responsible. When Tsunade gambles, Shizune immediately focuses on recovering their money and securing food and lodging. Her usual gentle demeanor gives way to determined urgency whenever Tsunade’s debts threaten their survival. 8 10

Although Sōji’s talent sometimes discourages her, Shizune continues training and working steadily. Her faith in Tsunade is unwavering; she insists that Tsunade never truly abandoned medicine because she continues teaching, researching, and saving people despite her trauma. 13 14 27

Despite her embarrassment over Tsunade’s debts, Shizune gradually finds enjoyment in the group’s companionship. She cautiously drinks with Tsunade at Iwamiya and later buys souvenirs for everyone during their stay in the Land of Gold. 18 32

Shizune’s concern for Sōji extends to his training and travel plans. She worries about the dangers of his proposed return to the Land of Water and urges him to choose a safer place to practice after he descends from a ship’s mast using weight-altering ninjutsu. Although astonished when Tsunade offers to teach him her core secret techniques, Shizune ultimately approves because she believes his growth and dedication have earned that trust. 16 33 34

She is observant of Tsunade’s emotional state but reluctant to intrude. When she sees Tsunade resting against Sōji’s chest, she recognizes a relaxation she has never previously witnessed in her mentor. Her feelings are complicated by Sōji’s many secrets, but she tries to set those concerns aside and pretend she has seen nothing; her involuntary cry of surprise prevents her from doing so. 170

Shizune studies Chakra Enhanced Strength under Tsunade and states that she has trained in it for two or three years by the time Sōji demonstrates the technique. Her control is precise, but she struggles because she distributes chakra too evenly instead of compressing it into a single point. 13 14

Shizune becomes the key intermediary between Sōji and Tsunade. She recognizes the value of his medical knowledge, believes in his wish to survive, and secures Tsunade’s approval to treat him. Her work contributes to the complete cure of his bloodline disease after a year of treatment. 5 6 7

Tsunade permits Shizune to take the case on the conditions that treatment remain nearby, every medication and treatment plan receive her review, and half the agreed two million ryō be paid in advance. The case becomes the most fulfilling and challenging period of Shizune’s medical career up to that point, and Tsunade explicitly praises her work when Sōji’s regeneration stabilizes. 6 7

Journey with Tsunade and Sōji

Shizune travels with Tsunade and Sōji aboard the Yamada Trading Company’s merchant ship. She serves as the group’s responsible caretaker, manages Tonton, worries about money, and helps Sōji begin formal medical-ninjutsu training. 8 10 12

She provides her old medical-ninjutsu notes and hand-drawn chakra-pathway charts, beginning with the thirty-six paired co-assistance points of the chakra pathways. Later, while training alongside Sōji in Chakra Enhanced Strength, she is dismayed when his first attempt surpasses her years of practice. Tsunade tells her to stop comparing herself to his exceptional talent and continue working toward her own benchmark. 12 13 14

Land of Gold Medical Crisis

During the conflict involving the Fourth Raikage and the Xuan Yuan Corps, Shizune assists with surgery and protects Tsunade’s reputation when her hemophobia resurfaces. She publicly explains that Tsunade’s medical ideals continue through those she teaches, including Sōji. 25 27

After Sōji protects Tsunade from the Xuan Yuan Corps, Shizune thanks him and later proves her own growth by successfully operating on Amai. 30 31

Her defense of Tsunade frames the Will of Fire as a continuing process in which one person inspires another. She argues that Tsunade’s teaching, research, willingness to help, and refusal to leave the operation despite her fear demonstrate that her medical vocation has never disappeared. 27

Farewell

When Sōji leaves to travel through the Five Great Nations, Shizune breaks down in tears and gives him repeated advice to stay safe, eat properly, rest, and summon them if he is in danger. 35

She later finds Tsunade at the fork outside Tanzaku Town and sees that Sōji’s parting gesture has left her mentor quietly moved. 36

Reunion and Return to Konoha

When Sōji reunites with Tsunade in Tanzaku Town, Tsunade sends Shizune outside with Tonton before confronting him privately. On returning, Shizune catches Tsunade leaning against Sōji and cries out in surprise, causing her mentor to spring away in embarrassment. Sōji calmly greets her after their long separation. 169 170

Shizune remains with Tonton during the subsequent discussion of Orochimaru’s anticipated attack and Tsunade’s plans for returning to Konoha. She then accompanies Tsunade, Sōji, and the Land of Water delegation to the village. Before Tsunade departs after delivering Hashirama’s Wood Release manual to Sōji, Shizune reminds her that it is time to leave for the carriage at the relay station. 170 171
