Background / History

Tsunade is the granddaughter of the First Hokage, Hashirama Senju, and the grand-niece of the Second Hokage, Tobirama Senju, whom she also addresses as a grandfather. She carries the royal bloodline of the Senju Clan and has Uzumaki ancestry through her grandmother, Mito Uzumaki 3 88 132 319 592. Unlike her original storyline, she did not develop hemophobia after Dan's death or flee the village at that time; she later developed hemophobia after witnessing Jiraiya die 1 591. During the Third Shinobi World War, she served as Konoha's premier Medical Saint, frequently rushing between front lines to save wounded soldiers 1 24. During the Second Shinobi World War, she had also developed antidotes to Chiyo's poisons and proposed assigning a medical ninja to every mission squad 3 313.

Her connection with Ryobu began at the Ninja Academy, where she initially bullied him. Their later affection was obstructed by her clan and their teacher because of his Uchiha heritage. After Nawaki's death, Ryobu was the only person to openly support her medical reform proposal at the Jōnin Council, while Jiraiya and Orochimaru remained silent 3 132 454. Ryobu subsequently stole medical research from her home to develop Chimera: Plunder Regeneration; Tsunade knew about the theft but did not expose him. Jiraiya's death while shielding her from Danzo reopened the trauma of Nawaki's death and caused her hemophobia. She later received the Sage Talisman used to resurrect Jiraiya 530 531 592 595.

The later Rōran-linked storyline depicts Tsunade as Konoha's Fifth Hokage, making decisions about Naruto's missions and the threat of Akatsuki. Its account of her position and her response to Ryobu's overtures differs from the Amegakure storyline and should not be conflated with her marriage and leadership there 828 907 939. In this storyline, she remains devoted to the memory of Dan and has not accepted another lover, while treating Kushina's son Naruto as family 841.

Tsunade is proud, strong-willed, and fiercely independent, possessing the immense dignity of the Senju lineage 3 138. Though initially haughty and confident in her status, she possesses a deeply compassionate side regarding the welfare of injured ninjas and orphans 7 13. She is prone to stubbornness and strong emotional convictions, torn between her historical ties to the Will of Fire and her genuine love for Liao Wu 138 527. Despite her occasional sharp temper and gambling habits, she possesses the bearing of a general when managing administrative and village affairs 8 454. Her divided loyalties caused severe despair, but she resolved to define her life by her own choices rather than deny her past 307.

Her outward strength conceals a pronounced vulnerability to losing loved ones. Orochimaru recognized this after Nawaki's death, and Nonō later described her formidable power as masking a fragile heart. Jiraiya's death brought this vulnerability to the surface, causing overwhelming guilt and a disabling fear of blood; Tsunade nevertheless resolved to identify and cure the cause of her condition 8 531 591.

Her compassion does not always override national allegiance. In the later storyline's account of the Sannin meeting Nagato, Yahiko, and Konan, she gave the war orphans food but would not teach foreign children ninjutsu that might eventually threaten Konoha 890 928.
