Background / History
War Trauma and Withdrawal
Tsunade is the granddaughter of Hashirama Senju and Mito Uzumaki. She knew Madara Uchiha as a child and personally witnessed her grandfather's Wood Release. Her younger brother, Nawaki, later died on the battlefield 41 106 141 142.
The Second Shinobi World War left her deeply traumatized, withdrawn, and reluctant to fight. Her hemophobia persisted for more than a decade, severely limiting her battlefield usefulness despite her strength and medical expertise. Hiruzen recognized that her losses had also fostered antiwar sentiment 35 43 80.
Meeting Yūto
Tsunade first encounters Yūto when she catches him watching her at a bathhouse through binoculars. She breaks through the wall and strikes him hard enough to send him through three trees. After learning that he intends to donate his novel's royalties to the orphanage, she spares him and confronts Hiruzen over its seventy-percent funding cut. Hiruzen explains that he had recently entrusted the orphanage to Danzō, whom Tsunade immediately blames 23 24.
Nonō later persuades Tsunade to protect Yūto during his attempt to avenge the Elder by killing Dodai. Tsunade initially refuses, but Nonō threatens to expose her repeated gambling-related absences from work. On the battlefield, her hemophobia confines her to a supporting role, with responsibility for rescuing Yūto and Tsukimi if their attack fails 32 35.
Recovery and the Kumogakure Front
After Dodai's defeat, Yūto repeatedly provokes Tsunade while secretly hoping she will kill him. He also exploits her hemophobia by biting his finger, causing her to faint. His subsequent care—particularly his decision to keep his bloodied hands away from her—leaves her curious about the contrast in his behavior. She personally selects him for a medical-supply escort mission 36 39.
Kakuzu ambushes them after a casino visit. Tsunade refuses to abandon Yūto, whose determination to protect her reminds her of Nawaki. When Yūto suffers near-fatal injuries, the shock breaks her hemophobia. She defeats Kakuzu, heals Yūto, and decides to conceal her recovery so enemies will continue relying on outdated intelligence. She remains at the Kumogakure front to treat the wounded until peace is concluded 40 41 42 43 74.
Sunagakure Campaign
Hiruzen sends Tsunade to counter Chiyo's poison after Konoha's Poison Extraction Team proves unable to treat it. Although she initially invokes her hemophobia as an excuse, Hiruzen reveals that he already knows she has recovered. She agrees to go and insists that Yūto accompany her 80.
At the front, Tsunade immediately begins preparing antidotes. Having completed the treatment, she leads reinforcements to Outpost No. 7, strikes down Rasa, and forces Sunagakure's army to retreat. She summons Katsuyu to treat Yūto and kisses him as a reward for covering his comrades' withdrawal 83 84 93.
Madara's Attack and Her Confession
During the return journey from Tanzaku Town, Tsunade and her companions encounter the reincarnated Madara. She fights alongside Kisame, restrains Madara for Yūto's sealing attempt, and heals Yūto between exchanges. Their combined efforts fail to contain Madara, who breaks free and escalates to the Rinnegan and Susanoo 140 141 142 143.
Yūto pushes Tsunade out of the path of Madara's chakra blade and is cut in half. She attempts to heal him and confesses that she likes him, but cannot prevent his death. After Yūto returns to life and awakens his Mangekyō Sharingan, she again helps protect him during the battle. Later, she confirms that her confession was genuine, though she qualifies it as liking him “a little” 143 144 145 146.
Mangekyō Research and Later Years
Tsunade devotes herself to repairing the damage caused by Yūto's Mangekyō Sharingan. After 127 failed medicinal attempts, she travels with him to Kusagakure to find an Uzumaki woman with exceptional healing abilities. They rescue Yíngsháo, and Tsunade promises that she will no longer be harmed 182 183.
When Kusagakure hires Kakuzu to retrieve Yíngsháo, Tsunade supports Yūto's plan to punish the village's leaders through Hozuki Castle. She later heals Mui so he can help turn the Box of Ultimate Bliss conspiracy against those leaders. Back in Konoha, she studies Yíngsháo's constitution and develops eye drops intended to restore Yūto's Mangekyō vision 185 193 195 218.
During the Chunin Exams crisis, Tsunade supplies Yūto with several bottles of the eye drops, offers to fight Obito, and places key medical personnel on standby. In the finale, she expresses a desire to have a child of her own. Despite her poor opinion of the Hokage position, she supports Yūto's succession as the Fifth Hokage and offers him her grandfather's pendant 253 264 266.
Appearance
- Hair: Long and golden-blond, commonly worn in two ponytails; tied into a high ponytail while working in her laboratory 39 43 182.
- Build: A conspicuously curvaceous figure and large bust 72 81 184.
- Forehead mark: A diamond-shaped Yin Seal, from which black markings spread when she activates the Strength of a Hundred Technique 141.
- Combat clothing: A dark green uniform beneath a standard Konoha tactical vest during her departure for the Sunagakure front 81.
- Medical clothing: A lab coat, sometimes stained with medicine during prolonged research 46 182.
Personality
Tsunade is blunt, proud, and quick-tempered. She readily uses intimidation or physical force when embarrassed or angered, including striking Yūto over comments about her age and threatening Mitarashi Sen into keeping their kiss secret. She is particularly sensitive about the age gap between herself and Yūto 93 150.
Her reluctance to accept missions does not erase her commitment to patients. On reaching the Sunagakure front, she immediately abandons her complaints and demands access to the medical tent. Her determination to restore Yūto's eyesight likewise keeps her working through repeated failures and visible exhaustion 83 84 182.
She enjoys gambling and drinking, accumulates substantial gambling debts, and repeatedly skips work to indulge those habits. Nevertheless, she is willing to confront village leadership over harm to vulnerable people, as shown by her response to the orphanage's funding cuts 23 24 32 40 139.
Her attachment to Yūto includes jealousy and serious violations of his consent. She drugs him, conceals what she has done, and supplies drugs used by Pakura and Tsukimi in further incidents. These actions coexist with her genuine affection and sustained efforts to protect and treat him 74 99 117 170 182.
Abilities & Skills
Medical Ninjutsu
Tsunade serves as Konoha's chief medical ninja and treats both battlefield injuries and ordinary hospital patients 46 117.
- Mystical Palm Technique: Channels medical chakra into Yūto to stem bleeding during the fight with Kakuzu 42.
- Rapid wound treatment: Heals Pakura's injured waist and treats Yūto's severe injuries after combat 43 99.
- Clinical examination: Determines that Pakura is not pregnant 117.
- Limits: Cannot save Yūto after Madara bisects him. Her early medicines also lack the regenerative power needed to repair Mangekyō damage 143 182.
Monster Strength and Taijutsu
Her physical attacks can break hardened defenses, devastate terrain, and overpower high-level opponents 41 42 93.
- Breaks through Kakuzu's Earth Release hardening with a punch 41.
- Uses Heavenly Foot of Pain to drive half of Kakuzu's body into the ground 42.
- Destroys Kakuzu's separated masks; the resulting shock wave snaps trees within a hundred meters 42.
- Defeats Rasa with a punch during the relief of Outpost No. 7 93.
- Limits: Her strength and regeneration do not allow her to overcome Madara's Susanoo 141 143.
Yin Seal and Strength of a Hundred Technique
The Yin Seal supports her youthful appearance and releases spreading markings when she activates the Strength of a Hundred Technique 40 42 141.
- Uses the released technique during her recovery from hemophobia and subsequent assault on Kakuzu 42.
- Emerges unwounded from a crater during the battle with Madara while the technique is active 141.
- Volunteers to serve as a test subject for Yūto's Mangekyō ability because her regenerative technique makes her a safer subject than another person 146.
Katsuyu Summoning
Tsunade summons Katsuyu to provide healing assistance 93.
- Calls Katsuyu to treat Yūto after rescuing him from Rasa 93.
- Recognizes Katsuyu's rapid-healing ability but explains that a summoning creature's power cannot be used directly as a medicinal ingredient 182.
Antidotes and Pharmaceutical Research
Tsunade previously neutralized Chiyo's poisons during the Second Shinobi World War and is again called upon when Chiyo's new poison defeats Konoha's existing treatment methods 80.
- Prepares antidotes from formulas she has memorized 83.
- Completes the Sunagakure-front antidote before joining the relief of Outpost No. 7 93.
- Works with Yíngsháo to produce eye drops for Yūto's damaged Mangekyō vision 195 218 253.
Battlefield Analysis
Tsunade understands the value of medical intelligence and concealed weaknesses 43 47.
- Keeps her cured hemophobia secret to mislead enemies who know about the condition 43.
- Identifies enemy control as the likely cause of Konoha shinobi's unexplained self-destruction 47.
- Limits: Her attempts to disrupt the affected Hyuga ninja's chakra and break the influence through pain fail; Fugaku's Sharingan resolves the immediate threat 47.
Equipment / Items
- Ancestral pendant: A treasure inherited from her grandfather, previously given to Nawaki. She offers it to Yūto when supporting his appointment as Fifth Hokage 266.
- Laboratory equipment: Uses a microscope, test tubes, and written experimental records while developing treatment for Mangekyō damage 182.
- Mangekyō eye drops: Stores the jointly developed medicine in a locked cabinet and gives Yūto several bottles before the anticipated attack on Konoha 253.
