Background / History
Senju heritage and departure
Tsunade is Hashirama Senju’s granddaughter and Nawaki’s elder sister. Neither sibling uses a surname, a detail Shinsei considers when examining the possibility that the Senju abandoned their clan name and integrated into Konoha 3.
Shinsei’s grandfather identifies Nawaki’s death and the Senju Clan’s decline as the suspected principal reasons for Tsunade’s departure from Konoha. Nawaki’s death was suspicious: his internal organs had been removed. Tsunade later connects her refusal to permit human experimentation with memories of her brother 21 75.
Her grief persists in private. While asleep, she mistakes Shinsei for Nawaki, promises to protect him, and cries out in distress. Naruto eventually helps her overcome her fear of blood; Shinsei’s Visualization Method and her reunion with Mito subsequently help restore her mental balance 76 216.
Return and appointment as Fifth Hokage
Jiraiya and Naruto bring Tsunade back to Konoha during the search for a successor to Hiruzen Sarutobi. She receives an enthusiastic welcome at the village gates 58 59.
At Shinsei’s grandfather’s request, she accepts Shinsei as her apprentice. Shizune reports gambling debts of 130 million ryo, and Tsunade persuades Shinsei to give her 200 million ryo ostensibly to repay them. She instead heads toward further gambling, convinced she can win enough to settle the debt and retain a surplus 60 61.
After formally becoming Fifth Hokage, Tsunade gives Shinsei medical, sealing, and chakra-theory texts. She also arranges his Katsuyu summoning contract and recruits him from the Anbu as her assistant for special matters, leaving Shizune primarily responsible for paperwork 61.
Meeting her younger self
In another Shinobi World corresponding to Konoha Year 39, Shinsei and Hinata encounter a younger Tsunade who has not yet received the Sannin title. After accepting their explanation, she takes her other self’s disciples under her protection 97 99.
The younger Tsunade provides detailed sealing instruction, protects Hinata from Hyuga demands that she return to the clan or receive the Caged Bird Seal, and fights alongside the Konoha group in the Land of Rain 98 99 116.
Shinsei later brings her, Minato, and Kushina into his own world. The two Tsunades initially clash over age and appearances, but establish a shared working arrangement. They alternate Hokage duties and exchange knowledge, allowing the younger counterpart to improve rapidly 122 123 125 129.
Opposition to the Akatsuki
The younger Tsunade accompanies the delegation to the Land of Water after winning a bet over which counterpart will go. During the confrontation with Pain, she summons Katsuyu to protect the shinobi from a colossal tsunami, regenerates her injuries, and engages Pain’s summons and ninjutsu-absorbing body 148 164 166.
At the Five Kage Summit, Tsunade proposes an elite Rapid Response Force rather than a mass allied army, arguing that ordinary shinobi cannot withstand Pain’s techniques. Mei Terumi, Chiyo, and Ōnoki support the proposal, and she nominates Shinsei as its captain 195.
Confronting Root and rebuilding Konoha
Tsunade refuses increased funding for Danzo and insists that Root transfer its ninja to the Anbu if it cannot support them. When Danzo disappears, she leads Konoha’s leadership into Root, orders its members detained and its records secured, and has Danzo declared an S-rank missing-nin 213 254.
Despite her personal reservations about the Uchiha, she guarantees equal treatment to clansmen considering relocation from the other world. After approximately thirty arrive, she assigns Sasuke and Shinsei responsibility for settling them in the Uchiha compound 253 290 293.
Front-line leadership and succession
Tsunade insists on personally joining the operation against Madara in Amegakure, despite opposition from Jiraiya, Tobirama, and numerous jonin. With Shinsei’s support, she maintains her decision and appoints Jiraiya Acting Hokage during her absence 344.
She gradually transfers authority to Shinsei, then requires him to gain experience teaching a team before succeeding her. Once he demonstrates the ability to handle matters involving the Ten-Tails, she gives him six months to prepare for office 338 378.
After Shinsei becomes Hokage, Tsunade relinquishes power without attempting to rule through him. She remains in Konoha, while Shizune continues serving as the new Hokage’s assistant 416.
Later crises and support for Shinsei
Both Tsunades help Minato and Sasuke intercept dangerous meteorites when the shattered moon threatens the Shinobi World 459.
During Shinsei’s year-long disappearance, Tsunade worries about the clans’ dissatisfaction with the vacant Hokage position. When he returns with Kaguya and their infant sons, she strikes him for leaving without a message, listens to his account, and warns him to accept the consequences of revealing his family 554.
She subsequently defends his position against opposition within Konoha. When he considers resigning, she reminds him of his unfinished reforms and urges him to unify the Shinobi World rather than abandon his promise of peace 556.
Appearance
Tsunade returns to Konoha appearing to be in her twenties or thirties, with a mature figure and a stern, heroic expression. Her younger counterpart initially looks noticeably different, but the two later adjust their appearances until observers must distinguish them through habits and mannerisms 59 122 210.
- Hair: Long blonde hair, parted at the center; her younger counterpart also wears it in a ponytail 59 102 129.
- Forehead mark: A blue-violet mark centered between her brows, associated with her sealing techniques 59 214.
- Clothing: Introduced wearing a green robe; wears a white laboratory coat while conducting research 59 221.
- Sage Mode: The older Tsunade emits faint white light and develops a black lock of hair above her forehead, without facial sage markings 215 216.
Personality
Tsunade is blunt, domineering, and quick to answer provocation with physical force. She nevertheless takes pride in her students’ achievements and becomes fiercely protective when their safety is involved. She warns Shinsei about suspicious mission intelligence and dangerous experiments rather than treating his growing strength as a guarantee of safety 100 197 225 368.
Gambling, alcohol, and avoidance of paperwork are recurring weaknesses. She demands repeated raises, appropriates disciples’ mission rewards, and often leaves administrative work to Shizune or Shinsei. Her younger counterpart initially differs in how she responds to teasing, but increasingly adopts her older self’s habits 61 69 82 221 222.
Her public confidence conceals enduring grief over Nawaki and the Senju Clan. Meeting Mito brings out an unusually vulnerable side: she embraces her grandmother in tears and later shows a girl-like bashfulness while displaying Sage Mode 75 76 136 217.
As Hokage, she is more tolerant of village gossip than Hiruzen and does not suppress rumors even when they concern her. Her political outlook also changes: initially cautious about Shinsei’s ideas, she eventually concludes that Hashirama missed an opportunity for unification and supports Shinsei’s attempt to achieve lasting peace 153 172 395 416 556.
Abilities & Skills
Medical Ninjutsu
Tsunade is described as the Shinobi World’s foremost healer, with extensive knowledge of anatomy and cellular function 82 197 211.
- Heals Sakura’s wounds, although blood loss still requires hospitalization 69.
- Uses Poison Extraction Jutsu to remove toxins from Shinsei’s four-person group without preparing an antidote 82.
- Works with her counterpart to administer a cell-based treatment to Mito, healing hidden injuries and nearly doubling her chakra 289.
- Recognizes that further cell-solution research requires facilities beyond those available to her 221.
- Opposes experiments on living people; Shinsei seeks cloned experimental material because he expects her to reject such testing 75 88.
Chakra Enhanced Strength and Taijutsu
The Chakra Enhanced Strength Technique, also called Superhuman Strength, is one of Tsunade’s signature techniques 76 179.
- Combines powerful punches with enough speed to pressure Shinsei in close combat and fracture the training ground 102.
- Uses positioning and opponents’ momentum to prevent a group from attacking her simultaneously 129.
- Overpowers Shinsei, Sasuke, Sakura, Hinata, Minato, and Kushina in a taijutsu training match once she becomes serious 129.
- Before Sage Mode, physical speed is her principal combat weakness 213.
- With support from Shinsei’s Shadow Clone and Light-Weight Rock Technique, moves behind the Fourth Mizukage, pulverizes half his reincarnated body, and seals him 352.
Yin Seal, Yin-Yang Seal, and Regeneration
Tsunade’s sealing expertise supports both chakra storage and advanced healing 82 98 213.
- Yin Seal: Stores chakra for later release; its instruction involves numerous sealing formulas and requires Yin Release chakra 76 82.
- Yin-Yang Seal: Developed with Shinsei to store senjutsu chakra, making Sage Mode more practical in battle 98 213.
- Strength of a Hundred Technique: A regenerative technique she created to rapidly repair injuries 164 181.
- Repairs damaged cells and regrows hair lost during natural-energy training 214.
- Her younger counterpart cannot initially design the complex seal needed for the Dragon Vein’s natural energy and defers to Mito’s expertise 102.
Sage Mode
Tsunade learns Shinsei’s version of Sage Mode after visualization training improves her spiritual balance and chakra control 215 216.
- In its initial test, increases strength roughly fivefold and speed more than fourfold, while also improving resilience 215.
- Produces a massive ground fissure with a punch even without Chakra Enhanced Strength 215.
- Forces Shinsei to use his Sharingan and Storm Release cell stimulation to keep pace during sparring 215.
- Initially depends on Shinsei to supply natural energy; storing the resulting senjutsu chakra does not equal independent mastery of absorption 216 217.
- Allows her to overpower a Water Mirror copy that could previously match her ordinary strength 351.
Katsuyu Summoning
Tsunade summons Katsuyu and gives her disciples access to the summoning contract 61 129.
- Travels with Shinsei to Shikkotsu Forest to consult the Katsuyu Sage 83.
- Deploys Katsuyu’s divided bodies to protect and heal multiple allies 164.
- Uses small Katsuyu bodies as a communications network during the confrontation with Obito 204.
- Relies on Katsuyu to remove dangerous natural energy during early Sage Mode practice 214.
Body Pathway Derangement and Technique Development
Tsunade’s understanding of the body extends beyond healing into combat and ninjutsu research 211.
- Body Pathway Derangement: Converts chakra into signals that disrupt an enemy’s bodily control 211.
- Explains that developing Storm Release requires Shinsei to discover the correct Water–Lightning chakra ratio through experimentation 69.
- Suggests using sealing jutsu rather than direct compression to solve Shinsei’s water-mine problem, enabling the development of sealed explosive water spheres 222 223.

Trivia
- Anbu personnel privately distinguish the counterparts as “Tsunade No. 1” and “Tsunade No. 2,” using behavioral differences despite their matching appearances 210.
- After the older Tsunade learns Sage Mode, Jiraiya suggests “Katsuyu Sage.” Her younger self instead proposes “Gambling Sage of the Shinobi World” as their shared title 216.
- Tsunade’s rare gambling wins usually precede trouble, but games against her counterpart are an exception: neither woman’s victories produce the usual ominous effect 378.
